助产士全程导乐陪伴护理在无痛分娩产妇中的应用效果

Application Effect of Doula Accompanied Nursing by Midwives in Women with Painless Labor

目的探究助产士全程导乐陪伴护理在无痛分娩产妇中的应用效果。方法方便选取2021年1月—2023年5月丰县人民医院接收的88例无痛分娩产妇的研究对象,依据随机数表法分为对照组(44例,常规护理)和观察组(44例,助产士全程导乐陪伴护理),对比两组分娩情况、不良分娩结局、心理健康情况、产妇自我效能及满意度。结果观察组第一、二、三及总产程时间均短于对照组,产后24 h出血量低于对照组,差异有统计学意义(P<0.05)。观察组不良分娩结局发生率为4.55%,较对照组的18.18%更低,差异有统计学意义(χ^(2)=4.062,P<0.05)。观察组产后2 h及出院前焦虑、抑郁评分均较对照组更低,差异有统计学意义(P<0.05)。护理后观察组结果期望、自我效能期望评分及总分均较对照组更高,差异有统计学意义(P<0.05)。观察组护理总满意度为97.73%,较对照组的81.82%更高,差异有统计学意义(P<0.05)。结论助产士全程导乐陪伴护理模式的应用,可发挥缩短产程与调节产妇不良情绪及降低不良分娩结局发生率的作用。

Objective To explore the effect of doula accompanied nursing for midwives in painless labor.Methods A total of 88 women with painless delivery admitted to Fengxian People's Hospital from January 2021 to May 2023 were conveniently selected and divided into control group(44 cases with routine care)and observation group(44 cases with doula accompanied care by midwives)according to random number table method.The delivery status,adverse delivery outcomes,mental health status,maternal self-efficacy and satisfaction of the two groups were compared.Results The first,second,third and total labor time of observation group were shorter than that of control group,and the postpartum blood loss at 24 h was lower than that of control group,the difference was statistically significant(P<0.05).The incidence of adverse delivery outcomes in the observation group was 4.55%,which was lower than 18.18%in the control group,and the difference was statistically significant(χ^(2)=4.062,P<0.05).The anxiety and depression scores of the observation group were lower than those of the control group 2 hours after delivery and before discharge,and the differences were statistically significant(P<0.05).After nursing,the outcome expectation,self-efficacy expectation score and total score of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The total nursing satisfaction of the observation group was 97.73%,which was higher than that of the control group(81.82%),and the difference was statistically significant(P<0.05).Conclusion The application of doula accompanied nursing mode of midwives can shorten the labor process,regulate the bad mood of the women and reduce the incidence of adverse labor outcomes.
