透明质酸钠联合关节镜微创手术对老年膝骨关节炎患者膝关节功能及炎性因子水平的影响

Effects of sodium hyaluronate combined with arthroscopic minimally invasive surgery on knee joint function and inflammatory factors levels in elderly patients with knee osteoarthritis

目的探讨透明质酸钠联合关节镜微创手术对老年膝骨关节炎(KOA)患者膝关节功能及炎性因子水平的影响。方法选取2019年1月至2021年1月我院收治的158例老年KOA患者作为研究对象,采用随机数字表法将其分为对照组和观察组,各79例。对照组给予关节镜微创手术治疗,观察组在对照组基础上给予透明质酸钠治疗。比较两组的治疗效果。结果观察组的治疗总有效率为96.20%,显著高于对照组的84.81%(P<0.05)。治疗1、3、6个月,观察组的膝关节Lysholm评分高于对照组(P<0.05)。治疗后,观察组的白细胞介素-8(IL-8)、白细胞介素-17(IL-17)、环氧合酶-2(COX-2)水平低于对照组(P<0.05)。治疗后,观察组的总Ⅰ型胶原氨基端延长肽(TPⅠNP)、骨钙素(BGP)、骨保护素(OPG)水平高于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。结论透明质酸钠联合关节镜微创手术治疗老年KOA患者的效果显著,可改善膝关节功能、骨代谢情况,降低炎性因子水平及并发症发生率。

Objective To investigate the effects of sodium hyaluronate combined with arthroscopic minimally invasive surgery on knee joint function and inflammatory factors levels in elderly patients with knee osteoarthritis(KOA).Methods A total of 158 elderly KOA patients admitted in our hospital from January 2019 to January 2021 were selected as the research objects and divided into control group and observation group by random number table method,with 79 cases in each group.The control group was treated with arthroscopic minimally invasive surgery,and the observation group was treated with sodium hyaluronate on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 96.20%,which was significantly higher than 84.81%in the control group(P<0.05).After 1,3 and 6 months of treatment,the Lysholm score of knee joint in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of interleukin-8(IL-8),interleukin-17(IL-17)and cyclooxygenase-2(COX-2)in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of total N-terminal propeptide of typeⅠprocollagen(TPⅠNP),osteocalcin(BGP)and osteoprotegerin(OPG)in the observation group were higher than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Sodium hyaluronate combined with arthroscopic minimally invasive surgery in the treatment of elderly patients with KOA has a significant effect,which can improve knee joint function and bone metabolism,reduce the levels of inflammatory factors and the incidence of complications.
