自体富血小板血浆联合负压吸引技术治疗糖尿病足感染的效果

Effect of autologous platelet rich plasma combined with negative pressure suction technology on diabetic foot infection

目的探究自体富血小板血浆(PRP)联合负压吸引技术(VSD)治疗糖尿病足感染(DFI)的效果。方法选取2020年1月至2021年3月赣南医学院第二附属医院收治的60例DFI患者作为研究对象,随机分为观察组与对照组,每组30例。观察组行PRP+VSD治疗,对照组行VSD治疗,比较两组创面修复因子[血管内皮生长因子(VEGF)、表皮生长因子(EGF)、碱性成纤维细胞生长因子(bFGF)]、生命质量[世界卫生组织生命质量测定量表简表(WHOQOL-BREF)评分]、创面愈合时间、创面愈合率、临床疗效及不良反应发生情况。结果治疗前,两组VEGF、EGF、bFGF水平比较差异无统计学意义;治疗后,两组VEGF、EGF、bFGF水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。治疗前,两组日常活动、心理功能、生理功能评分比较差异无统计学意义;治疗后,两组日常活动、心理功能、生理功能评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组创面愈合时间短于对照组,创面愈合率高于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率为96.67%,高于对照组的73.33%,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论PRP联合VSD治疗DFI效果显著,能促进患者创面愈合,提高患者生命质量,无明显不良反应,值得临床推广应用。

Objective To explore the effect of autologous platelet rich plasma(PRP)combined with vacuum sealing drainage(VSD)on diabetic foot infection(DFI).Methods 60 patients with DFI admitted to the Second Affiliated Hospital of Gannan Medical College from January 2020 to March 2021 were selected as the research subjects,and they were randomly divided into the observation group and the control group,with 30 cases in each group.The observation group was treated with PRP+VSD,while the control group was treated with VSD.The wound repair factors(vascular endothelial growth factor[VEGF],epidermal growth factor[EGF],basic fibroblast growth factor[bFGF]),quality of life(summary of the World Health Organization quality of life measurement scale[WHOQOL-BREF]),wound healing time,wound healing rate,clinical efficacy and occurrence of adverse reactions were compared between the two groups.Results Before treatment,there were no significant differences in the levels of VEGF,EGF and bFGF between the two groups;after treatment,the levels of VEGF,EGF and bFGF of the two groups were higher than before treatment,and the observation group was higher than the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the scores of daily activities,psychological function and physiological function between the two groups;after treatment,the scores of daily activities,psychological function and physiological function of the two groups were higher than those before treatment,and the ob-servation group was higher than that the control group,and the differences were statistically significant(P<0.05).The wound healing time in the ob-servation group was shorter than that in the control group,and the wound healing rate was higher than that in the control group,and the differences were statistically significant(P<0.05).The total effective rate in the observation group was 96.67%,which was higher than 73.33%in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion PRP combined with VSD is effective in the treatment of DFI,which can promote wound healing and improve the quality of life of patients without obvious adverse reactions,which is worthy of clinical promotion and application.
