滋肾疏肝方联合中药溻渍治疗肾虚肝郁型卵巢早衰闭经临床观察

Clinical efficacy of Zishen Shugan Formula combined with Chinese medicine stain therapy on premature ovarian failure with amenorrhea of kidney liver depression type

目的观察滋肾疏肝方联合中药溻渍治疗肾虚肝郁型卵巢早衰(POF)闭经的临床疗效。方法将100例肾虚肝郁型POF闭经患者按照随机数字表法分为2组,对照组50例予激素替代治疗,治疗组50例在对照组治疗基础上应用滋肾疏肝方联合中药溻渍。2组均治疗3个月经周期。比较2组疗效;比较2组治疗前后中医证候评分变化;比较2组治疗前后T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)变化;比较2组治疗前后血清卵泡刺激素(FSH)、黄体生成激素(LH)、雌二醇(E_(2))变化;比较2组治疗安全性。结果治疗组48例、对照组46例最终完成试验。治疗组总有效率91.67%(44/48),对照组总有效率71.74%(33/46),治疗组疗效优于对照组(P<0.05)。2组治疗后中医证候各项评分及总评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均较本组治疗前升高(P<0.05),CD8^(+)较本组治疗前降低(P<0.05);治疗后治疗组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均高于对照组(P<0.05),CD8^(+)低于对照组(P<0.05)。2组治疗后血清FSH、LH水平均较本组治疗前降低(P<0.05),E_(2)水平较本组治疗前升高(P<0.05);治疗后治疗组血清FSH、LH水平均低于对照组,E_(2)水平高于对照组(P<0.05)。2组安全性比较差异无统计学意义(P>0.05)。结论滋肾疏肝方联合中药溻渍治疗肾虚肝郁型POF闭经,能改善患者临床症状,提高治疗效果,可能与改善细胞免疫功能、纠正性激素水平紊乱等因素有关。

Objective To observe the clinical efficacy of Zishen Shugan Formula combined with Chinese medicine stain therapy on premature ovarian failure(POF)with amenorrhea of kidney liver depression type.Methods One hundred POF patients with amenorrhea of kidney liver depression type were randomly assigned into the treatment group(n=50,2 cases dropped off)and the control group(n=50,4 cases dropped off).All patients were managed by hormone replacement therapy,and those in the treatment group were additionally treated with Zishen Shugan Formula combined with Chinese medicine stain therapy.Treated for 3 menstrual cycles,the aim was to compare traditional Chinese medicine(TCM)syndrome score,t lymphocyte subsets CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),serum follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2)).The curative effect and safety were assessed.Results The overall effective rate in the treatment group was better than that in the control group(91.67%[44/48]vs 71.74%[33/46],[P<0.05],respectively).After treatment,TCM symptom scores in groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).Significantly increased CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and decreased CD8^(+)were found in the two groups than those before treatment(all P<0.05),and the different was significant between groups for the above indexes(all P<0.05).Significantly decreased FSH and LH,and increased E_(2) were detected in the two groups(all P<0.05),and the treatment group was superior to the control group for FSH,LH,E_(2)(all P<0.05).None of the safety differed significantly between groups(P>0.05).Conclusion For POF patients with amenorrhea of kidney liver depression type,Zishen Shugan Formula combined with Chinese medicine stain therapy can improve clinical symptoms and treatment effects by improving cellular immune function and correcting sex hormone level disorders.
