局部药物联合牙周治疗对口腔扁平苔藓的影响

Effect of topical ointment combined with periodontal therapy on oral lichen planus

目的分析局部药物联合牙周治疗对口腔扁平苔藓(OLP)的影响。方法将40例符合标准的OLP患者随机分为2组,每组20例,对照组采用局涂曲安奈德口腔软膏治疗,试验组采用局涂曲安奈德口腔软膏、西帕依固龈液联合牙周洁刮治术治疗。比较2组治疗1、3、6个月后的疗效,并在治疗前后采用焦虑自评量表(SAS)、抑郁自评量表(SDS)、口腔健康影响程度量表中文版(OHIP‑14)进行问卷调查,分析2组牙周指数、疗效及对心理生活质量的差异。结果试验组疗效高于对照组(P<0.05),治疗后2组牙周指数、SAS、SDS、OHIP‑14评分存在差异(P<0.05),局涂曲安奈德口腔软膏、西帕依固龈液联合牙周洁刮治术治疗通过改变口腔微环境来改善疗效及患者心理生活质量。结论局部药物联合牙周治疗可提高OLP疗效并减轻患者心理障碍。

Objective To analyze the effect of topical ointment combined with periodontal therapy on oral lichen planus(OLP).Methods Forty OLP patients were randomly assigned to 2 groups(n=20).The control group was treated with triamcinolone acetonide(TA)oral ointment,and the experimental group was treated with TA,Xipayi mouth‑wash,and periodontal scaling.The therapeutic effect of the two groups was compared at 1,3,and 6 months after treatment.A questionnaire survey was performed before and after treatment,including self‑rating anxiety scale(SAS),self‑rating depression scale(SDS),and oral health impact scale(OHIP‑14 Chinese version).The differences of periodontal index,therapeutic effect,and psychological quality of life between the two groups were analyzed.Results The efficacy of the experimental group was significantly better than that of the control group(P<0.05).After treatment,there were significant differences in the periodontal index and the scores of SAS,SDS,and OHIP‑14 between the two groups(P<0.05).Treating with TA,Xipayi mouth‑wash,and periodontal scaling could improve the therapeutic effect and psychological quality of life of patients by changing the oral microenvironment.Conclusion Topical application of ointment combined with periodontal therapy can improve the therapeutic effect and alleviate psychological barriers in OLP patients.
