帕金森病合并认知障碍患者的重复经颅磁刺激结合虚拟现实技术治疗效果

Analysis of the therapeutic effect of repetitive transcranial magnetic stimulation combined with virtual reality technology in patients with Parkinson’s disease and cognitive impairment

摘要 目的:探讨帕金森病(PD)合并认知障碍患者实施重复经颅磁刺激(rTMS)结合虚拟现实(VR)技术的临床价值。方法:选取于2018年7月至2022年12月期间的PD患者,入组对象为医院收治的PD认知障碍患者,共计80例,根据随机数字表法分2组各40例,对照组(实行常规康复治疗联合rTMS)与观察组(实行rTMS结合VR技术治疗);观察对比两组的认知功能、运动功能、日常生活能力、生活质量。结果:治疗后,观察组蒙特利尔认知功能评估量表(MoCA)为(25.76±2.95)分,简易精神状态检查量表(MMSE)为(27.05±2.58)分,对照组MoCA为(20.86±3.17)分,MMSE为(23.41±3.66)分,两组MoCA、MMSE评分均升高,观察组评分高于对照组(t=7.156、5.141,P<0.05);治疗后,观察组UPDRS-Ⅲ评分为(25.98±4.17)分,对照组UPDRS-Ⅲ评分为(28.15±2.36)分,两组UPDRS-Ⅲ评分均低于治疗前,观察组低于对照组(t=2.864,P<0.05);治疗后,观察组生活活动能力量表(ADL)评分为(60.28±8.25)分,治疗组ADL评分为(55.12±9.63)分,两组ADL评分高于治疗前,观察组高于对照组(t=2.573,P<0.05);治疗后,观察组PDQ-39评分为(24.87±3.15)分,对照组组PDQ-39评分为(27.63±4.35)分,两组PDQ-39评分均呈不同程度降幅,且观察组降幅大于对照组(t=3.250,P<0.05)。结论:针对PD合并认知功能障碍患者实施rTMS结合VR技术治疗,对其认知功能改善具有显著作用,同时有利于改善运动功能并提高日常生活能力,以保障患者良好生活质量。 Objective To explore the clinical value of repetitive transcranial magnetic stimulation(rTMS)combined with virtual reality(VR)technology in patients with Parkinson’s disease(PD)and cognitive impairment.Methods From July 2018 to December 2022,a total of 80 patients with cognitive impairment of PD admitted to the hospital were collected and filed.They were divided into two groups according to the"random number table"method,with 40 cases in each group.The control group(treated with routine rehabilitation therapy combined with rTMS)and the observation group(treated with rTMS combined with VR technology);Observe and compare the cognitive function,motor function,daily living ability,and quality of life between the two groups.Results After treatment,montreal cognitive assessment(MoCA):(25.76±2.95),mini mental state examination(MMSE):(27.05±2.58)in the observation group,MoCA:(20.86±3.17),MMSE:(23.41±3.66)in the control group,MoCA and MMSE scores increased in both groups,and scores in the observation group were higher than those in the control group(t=7.156,5.141,P<0.05);after treatment,unified parkinson’s disease rating scale(UPDRS-Ⅲ)score of the observation group:(25.98±4.17),UPDRS-Ⅲscore of the control group:(28.15±2.36),UPDRS-Ⅲscore of both groups was lower than that before treatment,and that of the observation group was lower than that of the control group(t=2.864,P<0.05);after treatment,activity of daily living scale(ADL)score of the observation group:(60.28±8.25),ADL score of the treatment group:(55.12±9.63),ADL scores of both groups were higher than before treatment,and the observation group was higher than the control group(t=2.573,P<0.05);after treatment,PDQ-39 score of the observation group:(24.87±3.15),and PDQ-39 score of the control group:(27.63±4.35),and the two groups’PDQ-39 scores showed different degrees of decrease,and the decrease was greater in the observation group than in the control group(t=3.250,P<0.05).Conclusion The combination of rTMS and VR technology treatment for patients with PD combined with cognitive impairment has a significant effect on improving their cognitive function,as well as improving motor function and daily living ability,to ensure a good quality of life for patients.
