住院患者出院随访系统的设计与实现

Design of a Post-Discharge Follow-up System for Inpatients

针对传统随访系统存在随访对象单一、覆盖范围小等问题,本文提出一种住院患者出院智能随访系统设计方案。该方案基于高内聚、低耦合设计方法,包含应用层、业务逻辑层、数据访问层、业务基础层、系统数据、外部连接;系统功能包含多用户管理模块、基于不同病区患者多维数据和知识库为医护人员提供智能决策。所提智能系统实现“以患者为中心”理念,提升患者诊疗体验,改善治疗效果。

In response to the limitations of traditional follow-up systems,such as their narrow focus on a single target and limited coverage,this paper presents a design proposal for an intelligent post-discharge follow-up system specifically designed for hospitalized patients.The proposed solution adopts a highly cohesive and loosely coupled design approach,incorporating various layers including the application layer,business logic layer,data access layer,business foundation layer,system data,and external connections.The system's functionalities encompass a multi-user management module and leverage multidimensional patient data and a knowledge base specific to different wards,providing healthcare professionals with intelligent decision-making capabilities.The intelligent system proposed in this paper embodies the philosophy of"patient-centered care"and aims to enhance the patient's diagnosis and treatment experience while improving treatment outcomes.
