儿童颅脑集成MRI的图像质量研究

The Study of Image Quality at Synthetic Brain MRI in Children

摘要 目的对比分析儿童颅脑MRI的集成序列与常规序列的图像质量。方法对40例儿童进行头部常规序列及集成序列MRI扫描,并按年龄分为A组(<2岁)和B组(>2岁)。比较常规序列T1FLAIR、T2WI、T2FLAIR图像与集成序列重建Syn T1FLAIR、Syn T2、Syn T2FLAIR的灰质、白质及脑脊液的信号强度及对比度,并对A组、B组两序列间的总体图像质量、灰白质分界清晰度、病变显著性以及伪影评分进行比较。结果Syn MRI灰质、白质和脑脊液的对比度与Con MRI有统计学差异;Syn T1FLAIR、T2灰白质与脑脊液的相对对比度与Con图像有统计学差异(P<0.05);两组Syn T1FLAIR和T2的总体图像质量均可被接受,对于T1FLAIR图像,A组集成图像灰白质分界评分较常规图像高(P<0.05),B组集成图像的病变显著性较常规图像有差异(P<0.05);对于T2WI图像,两组常规图像和集成图像的灰白质分界无显著差异(P>0.05),B组集成序列的病变显著性差异较常规序列高(P<0.05);对于T2FLAIR图像,A组常规图像的灰白质分界较集成图像更清晰(P<0.05),而B组无明显差异(P>0.05),两组序列图像的病变显著性无明显差异(P>0.05);集成序列的伪影评分均高于常规序列(P<0.05)。结论儿童脑部集成序列的T1FLAIR和T2WI图像可以常规用于临床诊断,并在一定程度上优于常规序列。但Syn T2FLAIR序列的质量普遍低于常规序列。在日常工作中可在集成序列的基础上加扫常规T2FLAIR。 Objective To compare and analyze the image quality of the synthetic brain MR in children compared with that of conventional images.Methods 40 children underwent synthetic and conventional imaging and divided into group A(<2 years)and group B(>2 years).Comparing the signal intensity and contrast of gray matter,white matter and CSF from conventional T,FLAIR、T,、T,FLAIR images and Syn T,FLAIR、T,、T,FLAIR images.The overallimage quality,gray and white matter demarcation definition,lesion significance,and artefact evaluation scores between the two sequences of group A and group B were also compared.Results The contrast of Syn MRI gray matter,white matter and CSF was statistically different from Con MRI;The relative contrast of gray/white matter and CSF in Syn T,FLAIR and T,was statistically different from the Con MRI(P<O.05);The overall image quality of Syn T,FLAIR and T,was acceptable for both groups,For the T,FLAIR images,score of gray/white matterdemarcation definition of Syn MRI in group A was higher than conventional images(P<0.05),The lesion significance of the Synthetic images in group B was different compared with the conventional images(P<0.05);For the T,-weighted images,No significant difference in gray/white matterdemarcation score between the conventional and integrated images(P>0.05),The lesion significance of the Synthetic images was higher than conventional sequencesin group B(P<0.05);For the T,FLAIR images,The gray and white matter demarcation of group A conventional images is clearer than the Synthetic images(P<0.05),which was no significant dfference in group B(P>0.05).No lesion significance between two sequence images(P>0.05);Synthetic sequence artifacts were higher than conventional sequences(P<0.05).Conclusion The T,FLAIR and T,-weighted images of pediatric brain Syn MRI can be routinely used for clinical diagnosis and are somewhat superior to conventional sequences.However,the quality of the Syn T,FLAIR is generally lower than the con MRI.In daily work,conventional T,FLAIR can be added on the basis of Syn-thetic sequences.
