探讨WAND高分辨MRI扫描技术在牙齿及牙周疾病检查中的可行性 被引量:1

摘要 目的探讨基于无线整合型MRI放大器(WAND)耦合头颈联合线圈高分辨MRI扫描技术在牙齿及牙周疾病检查中的可行性。方法对15例健康志愿者双侧下颌牙行常规(采用头颈联合线圈)MRI扫描和高清(WAND耦合头颈联合线圈)MRI扫描后获两组图像,比较两组图像的主观评分及客观评价,其主观评分包括对下颌管、下牙槽神经血管束及牙髓腔(牙髓、根管及根尖)等细微结构的显示情况;客观评价包括图像的信号强度、噪声、SNR及CNR。结果与常规组比较,高清组图像的信号强度、噪声、SNR及CNR差异均具有统计学意义(t=15.51,P<0.05;t=2.97,P<0.05;t=10.85,P<0.05;t=11.31,P<0.05),高清组图像信号强度为1595.19±414.20,较常规组提高41%;SNR为135.00±47.26,较常规组提高108%;CNR为125.23±45.22,较常规组提高131%;与常规组相比,高清组对下颌管、下牙槽神经血管束及牙髓腔(牙髓、根管及根尖)结构的主观评分明显提高且差异有统计学意义(P值均<0.05),高清组对各细微结构成像效果更清晰准确。结论WAND高分辨MRI扫描技术可提高牙髓腔及下牙槽神经血管束等超精细结构的显示能力,为牙齿及牙周疾病精准治疗提供可靠依据。 Objective To explore the feasibility of high-resolution MRI scanning technique based on WAND(wireless amplified nuclear MR detector)coupled head and neck coil in periodontal examination.Methods The bilateral mandibular teeth of 15 healthy young volunteers were scanned by MRI in routine group(only using conventional head and neck combined coil)and high definition group(WAND coupled with conventional head and neck combined coil).The subjective scores and objective evaluations of the two groups were compared,including the display of fine structures such as pulp cavity structure(pulp,root canal,and apical),mandibular canal,and inferior alveolar neurovascular bundle,and the objective evaluation included the Signal strength(SI),Standard Deviation(SD),the image signal-to-noise ratio(sign to noise ratio,SNR)and contrast-to-noise ratio(contrast to noise ratio,CNR)of the region of interest in the image.Results Compared with the conventional scanning group of head and neck joint coil,the images of SI,SD,SNR and CNR obtained by WAND coupled head and neck joint coil high-resolution MRI scanning were statistically different(t=15.51,P<0.05;t=9.27,P<0.05;t=10.85,P<0.05;t=11.31,P<0.05).The average SI of WAND high-resolution MRI scanning group was 1595.19±414.20,which was 41%higher than that of conventional scanning group.The average SNR was 135.00±47.26,which was 108%higher than that of the conventional scanning group.The average CNR was 125.23±45.22,which was 131%higher than that of the conventional scanning group.The subjective score showed that the WAND highresolution MRI scanning group had better imaging effect on the fine structures such as dental pulp cavity structure(pulp,root canal,and apical),mandibular canal,inferior alveolar neurovascular bundle,etc.,compared with the conventional scanning group.Conclusion The use of WAND coupled head and neck joint coil can improve the display ability of hyperfine structures such as dental pulp cavity and inferior alveolar neurovascular bundles,and provide a reliable basis for dental precision treatment of dental and periodontal diseases,which is worthy of clinical application.
