双能CT金属去伪影技术对髋关节金属假体伪影的临床评价 被引量:1

Clinical Evaluation of Hip Metal Prosthesis Artifacts by Dual-Energy CT Metal Artifacts Reduction Technology

摘要 目的本研究旨在评价双能CT(DECT)的3种金属去伪影方法在髋关节植入物金属伪影中的临床价值。方法回顾性分析接受髋关节置换术的20例患者的DECT图像,所有扫描均在双源128-MDCT扫描仪上获得。除了原始的DECT混合图像(De Mixed)外,还使用迭代去金属伪影重建算法(DE-iMAR)、虚拟单能成像(VMI)算法(DE Mono+)以及两种算法叠加(DE-iMAR Mono+)来重建DECT图像,这4组图像被匿名化并随机用于读者研究,由两名放射科医师采用4分法分别对4组图像进行图像质量评分,采用5分法从肌肉、膀胱、骨3个解剖区域进行伪影评分,并对SNR、AI值进行定量分析,以评估3种重建方法减少金属伪影的性能。结果4种重建方法图像伪影评分相比,DE-iMAR Mono+(1.79±0.53)与Mono+(2.52±0.53)、DE-iMAR(2.84±0.6)、De Mixed(4.47±0.51)均有显著性差异(P值均<0.05)。图像质量方面,DE-iMAR Mono+(3.65±0.49)与Mono+(2.21±0.41)、DE-iMAR(2.63±0.49)、De Mixed(2.05±0.52)差异有统计学意义(P值均<0.05)。整体图像质量评分的组内相关系数分别为0.819(P<0.001),伪影评分的组内相关系数为0.864(P<0.001)。此外,DE-iMAR Mono+组在定量分析中AI值(骨23.66±4.34、肌肉7.95±2.3、膀胱10.41±2.6)显著下降(P值均<0.05),同时SNR值(骨2.85±0.54、肌肉3.12±2.22、膀胱1.17±0.9)均高于其他组别(P值均<0.05)。结论DE-iMAR Mono+算法在减少髋关节金属假体伪影的性能最佳,具有良好的临床应用价值。 Objective The purpose of this study was to evaluate the clinical value of 3 metal artifact reduction methods of dual-energy CT(DECT)in patients with hip implant.Methods DECT images were retrospectively analyzed in 20 patients who underwent hip replacement and all scans were acquired on a dual-source 128-MDCT scanner.In addition to the original DECT mixed image(De Mixed),the DECT was reconstructed using an iterative metal artifact reconstruction algorithm(DE-iMAR),a virtual monoenergetic imaging(VMI)algorithm(DE Mono*),and a combination of the two algorithms(DE-iMAR Mono t).These four sets of images were anonymized and randomized for reader study.The image quality was scored by two radiologists by a 4-point scale and artifact was scored using a 5-point scale from muscle,bladder,and bone.SNR and AI values were calculated to evaluate the performance of the three reconstruction methods in reducing metal artifacts.Results There were statistically significant differences in artifact scores between DE-iMAR Mono*(1.79±0.53)and Mono*(2.52±0.53)or DE-iMAR(2.84±0.6)or De Mixed(4.47±0.51)and overall image quality scores between DE-iMAR Mono*(3.65±0.49)and Mono*(2.21±0.41)or DE-iMAR(2.63±0.49)or De Mixed(2.05±0.52)(all P<0.05).The intraclass correlation coefficient(ICC)of the overall image quality scores was 0.819,and the ICC of the artifact scores was 0.864(both P<0.001).In addition,the AI value of the DE-iMAR Mono*(bone 23.66±4.34,muscle 7.95±2.3,bladder 10.41±2.6)decreased significantly in the quantitative analysis(all P<0.05),while the SNR values(bone 2.85±0.54、muscle 3.12±2.22、bladder 1.17±0.9)were higher than other groups(all P<0.05).Conclusion The algorithm DE-iMAR Mono*showed the best performance in reducing hip metal prosthesis artifacts and was of good clinical value.
