Construction and Application of High-Risk Infants Management Platform

目的建设高危儿规范化管理平台,为实现区域高危儿系统化管理和早期干预促进提供平台支撑。方法基于广州市妇幼健康信息系统,以及国内外专家对高危儿分级分类管理的共识和规范,应用标准化数据集构建医疗机构-家庭-社区三位一体的高危儿系统化管理平台。结果在广州市基本形成了适用于超大城市的高危儿健康信息化管理模式,完成每月新增管理高危儿4000~5000人,成功实现双向转诊200余例,高危儿管理率达到90%,初步实现高危儿全程闭环管理。结论系统化的高危儿管理平台非常必要,可支持在超大城市进行高效率随访,为高危儿在不同层级卫生服务机构的转诊和随访提供有效衔接,提高了管理覆盖率和有效率,是我国高危儿管理的有益实践。

Objective Establishing a standardized management platform for realizing systematic management and early intervention of high-risk infants in the region.Methods Based on the Guangzhou maternal and child health information system,as well as the consensus and standards of domestic and international experts on graded classification management of high-risk infants,a three-in-one high-risk infants systematic management platform integrating medical institutions,families,and communities was constructed using standardized data sets.Results A high-risk infants health information management model suitable for large-scale cities has been established in Guangzhou,managing 4,000-5,000 new high-risk infants per month,successfully achieving over 200 two-way referrals,with a high-risk infants management rate reaching 90%,realizing comprehensive closed-loop management of high-risk infants.Conclusion It is necessary to establish a systematic high-risk infants management platform,which enables efficient follow-up in highly populated cities,provides effective linkage for referrals and follow-up at different levels of healthcare institutions for high-risk infants,improves management coverage and efficiency,and serves as a beneficial practice for high-risk infants management in China.
