尿蛋白水平与老年糖尿病患者血糖、血脂水平的相关性分析

Correlation between Urinary Protein Level and Blood Glucose and Lipid Levels in Elderly Patients with Diabetes

【目的】探讨尿蛋白水平与老年糖尿病患者血糖、血脂水平的相关性。【方法】回顾性分析2020年6月至2022年6月本院诊治的88例老年糖尿病患者的临床资料,根据患者尿蛋白水平分为蛋白尿组(n=46)和正常组(n=42)。比较两组患者一般资料及血糖、血脂水平;采用受试者工作特征(ROC)曲线分析血糖、血脂水平对老年糖尿病患者发生蛋白尿的预测价值;采用多因素Logistic回归性分析老年糖尿病患者发生蛋白尿的危险因素。【结果】蛋白尿组体重指数(BMI)≥24 kg/m^(2)、糖尿病病程≥5年、高血压、冠心病的患者占比及空腹血糖(FPG)、糖化血红蛋白(HbA1c)、甘油三酯(TG)、总胆固醇(TC)水平均显著高于正常组(P<0.05);ROC曲线分析结果显示:FPG、HbA1c、TG、TC预测老年糖尿病患者发生蛋白尿的曲线下面积分别为0.841、0.834、0.797、0.717;多因素Logistic回归性分析结果显示:BMI≥24 kg/m^(2)、糖尿病病程≥5年、高血压、FPG≥8.640 mmol/L、冠心病、HbA1c≥7.475%、TG≥1.365 mmol/L、TC≥4.870 mmol/L是老年糖尿病患者发生蛋白尿的危险因素(P<0.05)。【结论】老年糖尿病患者尿蛋白水平与血糖、血脂水平具有相关性,高血糖、血脂水平与糖尿病肾病相关。

【Objective】To explore the correlation between the level of urinary protein and the level of blood sugar and lipid in elderly patients with diabetes.【Methods】The clinical data of 88 elderly patients with diabetes diagnosed and treated in our hospital from June 2020 to June 2022 were retrospectively analyzed.The patients were divided into proteinuria group(n=46)and normal group(n=42)according to their urinary protein level.The general information,blood glucose and blood lipid levels of the two groups were compared.The ROC curve was used to analyze the predictive value of blood glucose and lipid levels on proteinuria in elderly patients with diabetes;Multivariate logistic regression was used to analyze the risk factors of proteinuria in elderly patients with diabetes.【Results】The proportion of patients with body mass index(BMI)≥24 kg/m^(2),duration of diabetes≥5 years,hypertension,coronary heart disease,fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),triglyceride(TG),and total cholesterol(TC)in the proteinuria group were significantly higher than those in the normal group(P<0.05);ROC curve analysis showed that the area under the curve of FPG,HbA1c,TG and TC predicting proteinuria in elderly diabetes patients was 0.841,0.834,0.797 and 0.717,respectively;Multivariate logistic regression analysis showed that BMI≥24 kg/m^(2),diabetes duration≥5 years,hypertension,coronary heart disease,FPG≥8.640 mmol/L,HbA1c≥7.475%,TG≥1.365 mmol/L,TC≥4.870 mmol/L were risk factors for proteinuria in elderly diabetes patients(P<0.05).【Conclusion】The level of urinary protein in elderly patients with diabetes is related to the level of blood sugar and blood lipid,and the level of hyperglycemia and blood lipid is related to diabetes nephropathy.
