新生儿缺氧缺血性脑病血清miR-124与miR-126的表达及临床意义

摘要 目的探讨新生儿缺氧缺血性脑病(HIE)血清微小核糖核酸(miR)-124、miR-126的表达及临床意义。方法选取2019-01—2023-02湘南学院附属医院收治的92例HIE患儿为HIE组,根据病情严重程度分为轻度组44例,中度组27例,重度组21例,另选取同期31名健康新生儿为对照组。采用Spearman相关性分析HIE患儿血清miR-124、miR-126表达与S100B、NSE水平的相关性,采用受试者工作特征(ROC)曲线分析血清miR-124、miR-126水平对新生儿HIE的诊断价值。结果与对照组比较,HIE组血清miR-124、miR-126表达降低,S100钙结合蛋白B(S100B)、神经元特异性烯醇化酶(NSE)水平升高(P<0.05)。轻度组、中度组、重度组血清miR-124、miR-126表达依次降低,S100B、NSE水平依次升高(P<0.05)。Spearman相关性分析显示,HIE患儿血清miR-124、miR-126表达与S100B、NSE水平呈负相关(rs=—0.757、—0.781、—0.800、—0.785,P<0.001)。ROC曲线分析显示,血清miR-124、miR-126表达单独和联合诊断新生儿HIE的曲线下面积分别为0.872、0.882、0.946。结论新生儿HIE血清miR-124、miR-126低表达,可能通过神经细胞损伤参与新生儿HIE过程,血清miR-124、miR-126表达联合诊断新生儿HIE的价值较高。 Objective To investigate the expression and clinical significance of serum micro ribonucleic acid(miR)-124 and miR-126 in neonatal hypoxic-ischemic encephalopathy(HIE).Methods Totally 92 children with HIE admitted to the Affiliated Hospital of Xiangnan University from January 2019 to February 2023 were selected as the HIE group,and were divided into 44 cases in the mild group,27 cases in the moderate group and 21 cases in the severe group according to the severity of the disease,and 31 healthy newborns in the same period were selected as the control group.Spearman correlation was used to analyze the correlation between serum miR-124 and miR-126 expressions and S100B and NSE levels in children with HIE,and receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of serum miR-124 and miR-126 levels on neonatal HIE.Results Compared with the control group,serum miR-124 and miR-126 expressions were decreased and S100 calcium-binding protein B(S100B)and neuron-specific enolase(NSE)levels were increased in the HIE group(P<0.05).Serum miR-124 and miR-126 expressions were decreased and S100B and NSE levels were increased in the mild,moderate and severe groups in that order(P<0.05).Spearman correlation analysis showed that serum miR-124 and miR-126 expressions in children with HIE were negatively correlated with S100B and NSE levels(rs=—0.757,—0.781,—0.800,—0.785,all P<0.001).ROC curve analysis showed that the area under the curve for serum miR-124 and miR-126 expression alone and in combination for the diagnosis of neonatal HIE was 0.872,0.882,and 0.946,respectively.Conclusion Serum miR-124 and miR-126 are lowly expressed in neonatal HIE and may be involved in the neonatal HIE process through neuronal cell injury,and the combined serum miR-124 and miR-126 expression is of high value in the diagnosis of neonatal HIE.
