虚拟现实技术改善儿童癫痫患者疼痛和焦虑的作用

摘要 目的分析虚拟现实(VR)技术在儿童癫痫患者疼痛和焦虑管理中的效果。方法采用随机对照试验,以2020-09—2022-09在新疆医科大学第一附属医院儿科就诊的98例患儿为研究对象,其中对照组48例,干预组50例。对照组接受常规治疗,干预组在常规治疗基础上接受VR干预。主要观察指标为面部疼痛量表修订版(FPS-R)、儿童焦虑相关障碍量表(SCARED)、儿童健康问卷(CHQ-PF50)评分、不良反应及VR干预满意度。结果对照组及干预组干预后FPS-R评分(2.86±1.35比4.25±1.50,1.75±1.00比4.24±1.65)、SCARED评分≥25分占比(33.3%比50.6%,49.4%比14.0%)均低于干预前,CHQ-PF50评分(65.78±18.67比49.50±15.64,78.35±20.14比50.18±16.05)高于干预前(P<0.05)。干预组干预后FPS-R评分(1.75±1.00比42.86±1.35)低于对照组,SCARED评分≥25分占比(14.0%比33.3%)低于对照组,CHQ-PF50评分(78.35±20.14比65.78±18.67)高于对照组(P<0.05)。对照组与干预组患儿疲劳、晕动病、焦虑、恐惧、再入院及癫痫发作频率增加对比差异均无统计学意义(P>0.05)。>90%的患儿对VR体验、VR干预对疼痛和焦虑的管理效果、VR干预的便利性及总体表示满意或非常满意。结论使用虚拟现实技术对癫痫儿童进行干预,可有效减少疼痛和焦虑,提高生活质量,且具有良好的安全性和可接受性,在儿童癫痫的临床管理中具有广阔的应用前景。 Objective To analyze the effect of virtual reality(VR)technology on the management of pain and anxiety in children with epilepsy.Methods This study adopted a randomized controlled trial design,taking children diagnosed at the Department of Pediatrics of the First Affiliated Hospital of Xinjiang Medical University from September 2020 to September 2022 as the subjects.A total of 98 children were included,with 48 in the control group and 50 in the intervention group.The control group received conventional treatment,and the intervention group received VR intervention on the basis of conventional treatment.The primary outcomes included the scores on the faces pain scale-revised(FPS-R),the screen for child anxiety related disorders(SCARED),and the child health questionnaire(CHQ-PF50),as well as the adverse reactions and satisfaction with VR intervention.Results The percentage of FPS-R(2.86±1.35 vs 4.25±1.50,1.75±1.00 vs 4.24±1.65)and SCARED score≥25 in the control group and the intervention group after intervention(33.3%vs 50.6%,49.4%vs 14.0%)were lower than before intervention,CHQ-PF50 scores(65.78±18.67 vs 49.50±15.64,78.35±20.14 vs 50.18±16.05)were higher than those before intervention(P<0.05).After the intervention,the FPS-R score(1.75±1.00 vs 42.86±1.35)of the intervention group was lower than that of the control group,the proportion of SCARED score≥25 points(14.0%vs 33.3%)was lower than that of the control group,and the CHQ-PF50 score(78.35±20.14 vs 65.78±18.67)were higher than those in the control group(P<0.05).There was no significant difference between the control group and the intervention group in fatigue,motion sickness,anxiety,fear,re-hospitalization and increased seizure frequency(P>0.05).More than 90%of the children were satisfied or very satisfied with the VR experience,the effect of VR intervention on pain and anxiety management,the convenience of VR intervention and overall.Conclusion The use of virtual reality technology to intervene in children with epilepsy may effectively reduce pain and anxiety,improve quality of life,and have good safety and acceptability.It has a broad application prospect in the clinical management of epilepsy in children.
