摘要
目的分析肝脏上皮样血管内皮瘤(hepatic epithelioid hemangioendothelioma,HEHE)的影像学特征。方法选取2014年1月—2022年12月在福建省肿瘤医院就诊并经穿刺后病理确诊的10例HEHE病例进行影像学特征分析。结果10例病例中,6例为CT检查,1例MRI检查,3例同时CT和MRI检查,3例正电子发射断层显像/X线计算机体层成像仪(positron emission tomography/computedtomography,PET/CT)检查。PET/CT肝脏病变标准摄取值(standard uptake value,SUV)3.6~7.0。6例患者出现双肺转移,1例肝门区及腹膜后淋巴结转移,1例门静脉右支癌栓。6例为多灶型,共130个病灶,其中右肝91个,左肝39个。包膜下病灶90个,包膜牵拉征48个。T_(2)WI、静脉期可见“靶征”“棒棒糖征”。2例为弥漫型,2例为肝右叶数个病灶融合,1例见钙化。结论HEHE的影像表现具有一定影像学特征。
Objective To analyze the imaging characteristics of hepatic epithelioid hemangioendothelioma(HEHE).Methods From January 2014 to December 2022,a total of 10 cases of HEHE were diagnosed in Fujian Cancer Hospital and confirmed by puncture pathology retrospectively to analyze their imaging characteristics.Results Of the 10 cases,6 cases had CT examination,1 case had MRI examination,3 cases had both CT and MRI examination,and 3 cases had positron emission tomography/computedtomography(PET/CT)examination.Liver disease standard uptake value(SUV)of PET/CT were 3.6-7.0.Six patients had double lung metastasis,1 case had hepatic portal area and retroperitoneal lymph node metastasis,and 1 case had right branch of the portal vein cancer thrombosis.Six cases were multifocal,with a total of 130 lesions,of which 91 lesions were in the right liver,39 lesions were in the left liver,90 lesions were subencoated lesions,and 48 lesions had enveloping signs."target sign"and"lollipop sign"can be seen in T_(2)WI and venous stage.Two cases were diffuse type,2 cases were fusion of several lesions in the right lobe of liver,1 case shows calcification.Conclusion The imaging performance of HEHE has certain imaging characteristics.
