血液灌流联合血液透析治疗尿毒症的效果及不良反应

摘要 目的分析尿毒症治疗中血液灌流联合血液透析治疗的实施价值及不良反应。方法将东莞市清溪医院血透室2021年2月—2023年1月收治的符合尿毒症指征的患者中抽取80例进行对比研究,遵循随机对照原则分为对照组和研究组,各40例。对照组实施血液透析治疗,研究组实施血液灌流联合血液透析治疗。分析上述治疗后患者转铁蛋白(transferrin,TRF)、甲状旁腺素(parathyroid hormone,PTH)、β_(2)-微球蛋白(beta-2-microglobulin,β_(2)-MG)、治疗有效率、炎症因子、不良反应的统计学差异。结果研究组治疗后TRF、PTH、β_(2)-MG各指标水平均低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者炎症因子水平的抑制效果均较高,且研究组各指标水平低于对照组(P<0.05);对照组不良反应发生率低于研究组(P<0.05);研究组总有效率高于对照组(P<0.05)。结论血液灌流联合血液透析的安全性及疗效均较高,治疗后可以帮助患者有效抑制炎症因子、TRF、PTH、β_(2)-MG水平,不良反应发生率较低,为维持性血液透析治疗的相关标准制定提供临床借鉴内容。 Objective To analyze the implementation value and adverse reactions of hemoperfusion combined with hemodialysis in the treatment of uremia.Methods A comparative study was conducted on 80 patients who met the indications of uremia admitted to hemodialysis room,Dongguan Qingxi Hospital from February 2021 to January 2023.According to the principle of randomized control,they were divided into control group and study group,with 40 cases in each group.The control group was treated with hemodialysis,and the study group was treated with hemoperfusion combined with hemodialysis.The comparative result of transferrin(TRF),parathyroid hormone(PTH),beta-2-microglobulin(β_(2)-MG),therapeutic efficacy,inflammatory factors and adverse reactions were analyzed.Results The levels of TRF,PTH andβ_(2)-MG in the study group after treatment were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the inhibitory effect of inflammatory factors in both groups was higher,and the level of indicators in the study group was lower than that in the control group(P<0.05).The incidence of adverse reactions in control group was lower than that in study group(P<0.05).The total effective rate of the study group was higher than that of the control group(P<0.05).Conclusion Hemoperfusion combined with hemodialysis has high safety and efficacy.After treatment,it can help patients effectively inhibit the levels of inflammatory factors,TRF,PTH andβ_(2)-MG,with a low incidence of adverse reactions,and provide clinical reference for the formulation of relevant standards for maintenance hemodialysis treatment.
