老年急性阑尾炎腹腔镜术后并发症的相关因素分析

Analysis of Related Factors of Complications After Laparoscopic Surgery for Elderly Acute Appendicitis

目的探讨影响老年急性阑尾炎腹腔镜术后并发症发生的相关因素。方法选择2021年1月—2023年1月在莆田学院附属医院就诊的122例老年急性阑尾炎腹腔镜手术患者,以患者是否发生术后并发症进行分组,其中30例患者发生并发症,纳入发生组;92例患者未发生并发症,纳入未发生组。针对两组患者进行单因素分析,寻找术后患者并发症发生危险因素。结果两组年龄、病程、病理类型、糖尿病、依从性比较,差异有统计学意义(P<0.05);两组性别、高血压比较,差异无统计学意义(P>0.05);多因素分析结果显示,年龄、病程、病理类型、糖尿病为老年急性阑尾炎腹腔镜术后并发症发生的危险因素(P<0.05);而依从性则是老年急性阑尾炎腹腔镜术后并发症发生的保护因素(P<0.05)。结论老年急性阑尾炎腹腔镜术后并发症发生率较高,而年龄、病程、病理类型、糖尿病为其危险因素,依从性则为保护因素,应尽早采取适当干预措施,减少并发症发生。

Objective To explore and analyze the related factors that affect the occurrence of complications after laparoscopic surgery for acute appendicitis in the elderly.Methods A total of 122 elderly patients with acute appendicitis undergoing endoscopic surgery in the Affiliated Hospital of Putian University from January 2021 to January 2023 were selected and divided into groups according to whether they had postoperative complications.Among them,30 cases were in the occurrence group,referring to those who had complications.There were 92 cases in the non-occurrence group,which refers to the patients without complications.Single factor analysis was conducted on the two groups of patients to identify risk factors for postoperative complications.Results There were significant differences in age,course of disease,pathological type,diabetes and compliance between the two groups(P<0.05).There was no statistically significant difference in gender and hypertension between the two groups (P > 0.05). Multivariate results showed that age, course of disease, pathological type and diabetes were the risk factors for complications after endoscopic surgery in elderly patients with acute appendicitis (P < 0.05). Compliance was a protective factor for postoperative complications of acute appendicitis in elderly patients (P < 0.05). Conclusion The incidence of complications after endoscopy in elderly patients with acute appendicitis is high, and age, course of disease, pathological type and diabetes are risk factors, while compliance is protective factor. Appropriate intervention measures should be taken as soon as possible to reduce the occurrence of complications.
