基于交互式视觉反馈的姿势控制训练对脑梗死患者神经功能的影响

Effects of posture control training based on interactive visual feedback on neurological function in patients with cerebral infarction

摘要 目的 探究交互式视觉反馈的姿势控制训练对脑梗死患者神经功能的影响。方法 分析2020年1月-2023年1月本院收治的129例脑梗死患者作为研究对象,将其随机分为观察组(63例)和对照组(66例);采用倾向性评分匹配法按1∶1匹配后2组均为62例患者;观察组采用交互式视觉反馈的姿势控制训练;对照组采用常规姿势镜反馈训练;比较匹配后2组患者的静态平衡能力、运动步行能力、躯干控制能力、蛋白指标水平、神经通路指标水平等的变化。结果 匹配后2组患者干预后静态平衡能力、运动步行能力、躯干控制能力得到明显改善(P<0.05),且观察组患者明显优于对照组患者(P<0.05);干预后2组患者蛋白指标半胱氨酸(Homocysteine, Hcy)、脂蛋白相关磷脂酶A2(Lipoprotein-associated phospholipase A2 Lp-PLA2)水平均显著降低(P<0.05),且观察组患者较对照组更低(P<0.05)。采用广义评估方程评价两种干预方式对患者神经通路指标水平的影响,干预后2组患者神经通路水平指标血管内皮生长因子(Vascular endothelial growth factor, VEGF),血管内皮生长因子受体2(Vascular endothelial production factor receptor 2,VEGFR2)水平均显著升高(P<0.05),且观察组患者较对照组更高(P<0.05)。结论 交互式视觉反馈的姿势控制训练对于改善脑梗死患者静态平衡能力、运动步行能力、躯干控制能力更佳;交互式视觉反馈的姿势控制训练可有效降低患者Hcy、Lp-PLA2蛋白的表达水平,提高患者神经通路水平。 Objective To investigate the effect of postural control training with interactive visual feedback on the neurological function of patients with cerebral infarction.Methods A total of 129 patients with cerebral infarction were admitted to our hospital from January 2020 to January 2023 as the research subjects,and they were randomly divided into an observation group(63 cases)and a control group(66 cases).The two groups were both 62 patients after matching according to the propensity score matching method in a 1∶1 manner.The observation group used postural control training with interactive visual feedback,and the control group used conventional postural mirror feedback training.We compared the changes in static balance ability,motor walking ability,trunk control ability,protein levels,and neural pathway index levels in the two groups after matching.Results The static balance,motor walking ability and trunk control ability of the two groups were significantly improved after the intervention(P<0.05),and the patients in the observation group were significantly better than the patients in the control group(P<0.05);the levels of protein indexes of homocysteine(Hcy)and lipoprotein-associated phospholipase A2(Lp-PLA2)were significantly reduced in both groups after the intervention(P<0.05),and the patients in the observation group were lower than the patients in the control group(P<0.05).A generalized assessment equation was used to evaluate the effects of the two interventions on the level of neural pathway indexes in the patients,and the level of neural pathway indexes vascular endothelial growth factor(VEGF),vascular endothelial production factor receptor 2(VEGFR2)levels were significantly elevated(P<0.05),and patients in the observation group were higher than those in the control group(P<0.05).Conclusion Postural control training with interactive visual feedback is better for improving static balance,motor walking ability,and trunk control in patients with cerebral infarction,and it can effectively reduce the expression levels of Hcy and Lp-PLA2 proteins,and improve the level of neural pathways in patients.
