B淋巴细胞在IgA肾病中的作用研究进展

摘要 IgA肾病(IgAN)是全球最常见的肾小球肾炎,是终末期肾病的主要原因。IgAN的发病机制尚不明确,学界普遍认为IgAN主要由半乳糖缺乏的IgA1(Gd-IgA1)和Gd-IgA1抗原抗体复合物在肾系膜中沉积引起。Gd-IgA1是IgAN发病的始动因素之一,但Gd-IgA1的起源尚不清楚。目前认为,Gd-IgA1沉积可能与B淋巴细胞的异常发育有关。B淋巴细胞在基因和黏膜免疫的共同作用下异常发育和分化,进而分泌Gd-IgA1,在IgAN的发病和疾病进展中发挥重要作用。基于B淋巴细胞在IgAN中的重要作用,其有望作为IgAN新的治疗靶点和监测疾病进展的生物标志物。 IgA nephropathy(IgAN)is the most common glomerulonephritis worldwide,and the main cause of end-stage kidney disease.The pathogenesis of IgAN is not yet clear,and the academic community generally believes that IgAN is mainly caused by the deposition of galactose-deficient IgA1(Gd-IgA1)and Gd-IgA1 antigen antibody complexes in the renal mesangium.Gd-IgA1 is one of the initiating factors of IgAN,but the origin of Gd-IgA1 is still unclear.At present,it is believed that the deposition of Gd-IgA1 may be related to the abnormal development of B lymphocytes.B lymphocytes undergo abnormal development and differentiation under the combined action of genes and mucosal immunity,leading to the secretion of Gd-IgA1,which plays an important role in the pathogenesis and progression of IgAN.Based on the important role of B lymphocytes in IgAN,it is expected to serve as a new therapeutic target for IgAN and a biomarker for monitoring disease progression.
