探讨血清同型半胱氨酸联合C肽对2型糖尿病早期肾损伤的诊断价值 被引量:2

摘要 目的观察血清同型半胱氨酸(homocysteine,Hcy)联合C肽对2型糖尿病早期肾损伤的诊断价值分析。方法选取2020年1月-2022年12月三明市第二医院收治的2型糖尿病患者121例为研究对象,以尿微量白蛋白/肌酐比值(urinary microalbumin/creatinine ratio,UACR)水平作为分组依据,UACR>30μg/mg的29例患者为早期肾损伤,纳入研究组,UACR≤30μg/mg的92例患者纳入对照组。比较两组患者Hcy及C肽指标水平,明确单独检测以及联合检测的诊断效能。结果研究组患者的Hcy水平高于对照组,而C肽指标低于对照组,差异有统计学意义(P<0.05)。联合诊断灵敏度、特异度及准确率均高于单一检测,差异有统计学意义(P<0.05)。结论针对于在2型糖尿病早期肾损伤的诊断中,予血清Hcy联合C肽检测,可帮助临床早期诊断,临床价值较高。 Objective To observe the diagnostic value of serum homocysteine(Hcy)and C-peptide in early renal in-jury of type 2 diabetes.Methods A total of 121 patients with type 2 diabetes admitted to Sanming Second Hospital from January 2020 to December 2022,urinary microalbumin/creatinine ratio(UACR)was used as the group basis.29 patients with UACR>30μg/mg had early kidney injury and were included in the study group,and 92 patients with UACR≤30μg/mg were included in the control group.Compared the levels of Hcy and C-peptide in the two groups of patients to determine their diagnostic value.Results The level of Hcy in the study group was significantly higher than that in the control group,while the level of C-peptide index was significantly lower than that in the control group,the difference were statistically significant(P<0.05).The combined diagnosis sensitivity,specificity and accuracy were all higher than the single test,and the difference was statistically significant(P<0.05).Conclusion For the diagnosis of early renal injury in type 2 diabetes,the detection of serum Hcy and C-peptide can help early clinical diagnosis,with high clinical value.
