综合营养干预联合同理心护理对腹腔镜结直肠癌根治术患者营养状况及CD-RISC评分的影响

Effect of Comprehensive Nutritional Intervention Combined with Em⁃pathic Nursing Care on Nutritional Status and CD-RISC Score of Patients Undergoing Laparoscopic Radical Surgery for Colorectal Cancer

目的综合营养干预联合同理心护理对腹腔镜结直肠癌根治术患者营养状况及心理韧性量表(Connor-Davidson Resilience Scale,CD-RISC)评分的影响。方法便利选取2020年1月—2022年1月期间于厦门大学附属第一医院接受腹腔镜结直肠癌根治术治疗的81例患者作为研究对象。2020年1月—2021年1月期间纳入的39例患者列为常规组,给予患者常规护理与普通营养支持。2021年2月—2022年1月期间纳入的42例患者列为综合组,在常规组基础上给予患者综合营养干预联合同理心护理。对比两组的营养状况、心理韧性及健康行为能力。结果护理1周,综合组的综合性营养评估表评分为(9.43±3.25)分低于常规组的(11.59±2.33)分,差异有统计学意义(t=3.414,P<0.05)。综合组血清白蛋白、血清前白蛋白、血清总蛋白、体质指数水平高于常规组,差异有统计学意义(P<0.05);综合组心理韧性量表的坚韧性、力量性、乐观性及总分均高于常规组,差异有统计学意义(P<0.05);综合组健康行为能力自评量表中的心理安适、营养、运动、健康责任评分均高于常规组,差异有统计学意义(P<0.05)。结论患者经腹腔镜结直肠癌根治术治疗后,予以其综合营养干预联合同理心护理能够有效改善其机体营养状态,强化心理韧性,同时提升健康行为能力。

Objective The effect of comprehensive nutritional intervention combined with empathic nursing on the nu⁃tritional status and Connor-Davidson Resilience Scale(CD-RISC)scores of patients undergoing laparoscopic radical surgery for colorectal cancer.Methods 78 patients who underwent laparoscopic radical surgery for colorectal cancer from January 2020 to January 2022 at the First Affiliated Hospital of Xiamen University were conveniently selected as study subjects.Thirty-nine patients included between January 2020 and January 2021 were classified as the conven⁃tional group,and the patients were given conventional nursing care and general nutritional support.Forty-two patients included between February 2021 and January 2022 were classified as the comprehensive group,and patients were given comprehensive nutritional intervention combined with empathetic care on the basis of the conventional group.The indicators of nutritional status,mental toughness,and health behavioral competence were compared between the two groups.Results At 1 week of care,the Scale for Comprehensive Nutritional Assessment score of the comprehen⁃sive group was points was(9.43±3.25)points,which was lower than that of the conventional group(11.59±2.33)points,and the difference was statistically significant(t=3.414,P<0.05).Serum albumin,serum prealbumin,serum total pro⁃tein,and body mass index levels in the comprehersire group were higher in the coventional group,and the differences were statistically significant(P<0.05).The CD-RISC of the comprehensive group had higher levels of resilience,strength,optimism and total scores than the conventional group,and the differences were statistically significant(P<0.05).The scores of psychological well-being,nutrition,exercise,and health responsibility in the Self-Rating Abili⁃fies for Health Practices Scale of the comprehensive group were higher than those of the conventional group,and the differences were statistically significant(P<0.05).Conclusion After laparoscopic radical surgery for colorectal cancer,comprehensive nutritional intervention combined with empathic nursing care can effectively improve the nutritional status of the body,strengthen the psychological resilience,and enhance the ability of health behavior.
