Meleis转移理论下延续性宣教对经尿道前列腺切除术后患者快速康复、出院准备度和自我效能的影响

摘要 目的探讨Meleis转移理论下延续性宣教对经尿道前列腺切除术后患者快速康复、出院准备度和自我效能的影响。方法选取2018年2月至2020年7月首都医科大学附属北京世纪坛医院收治的132例接受经尿道前列腺切除术治疗的患者作为研究对象。根据护理方式分为常规护理组(n=60)及延续性宣教组(n=72)。常规护理组采用常规护理干预,延续性宣教组在常规护理干预基础上接受Meleis转移理论下延续性宣教。比较两组的围术期指标(留置导尿管时间、住院时间、愈合时间和最大尿流率)、出院准备度[出院准备度量表(RHDS)、出院指导质量表(QDTS)]、自我效能。结果护理干预后,延续性宣教组留置导尿管时间、住院时间、愈合时间均短于常规护理组(P<0.05);干预后,两组的RHDS、QDTS评分均增加,且延续性宣教组高于常规护理组(P<0.05);干预后及出院2周后,两组自我效能评分增加,且延续性宣教组高于常规护理组(P<0.05)。结论给予经尿道前列腺切除术后患者Meleis转移理论下延续性宣教护理,可以加快患者的康复速度,提高患者出院准备度,完善患者的自我效能。 Objective To explore the effects of continuous education under Meleis transfer theory on rapid recovery,discharge readiness and self-efficacy in patients after transurethral resection of prostate.Methods A total of 132 patients undergoing transurethral resection of prostatic in Beijing Shijitan Hospital Affiliated to Capital Medical University from February 2018 to July 2020 were enrolled as the research objects.According to different nursing methods,they were divided into routine nursing group(n=60)and continuous education group(n=72).The routine nursing group received the routine nursing intervention,and the continuous education group received the continuous education based on the Meleis transfer theory.The perioperative indexes(catheter indwelling time,hospitalization time,healing time,maximum flow rate),discharge readiness[readiness for hospital discharge scale(RHDS),quality of discharge teaching scale(QDTS)],self-efficacy were compared between the two groups.Results After nursing intervention,catheter indwelling time,hospitalization time and healing time in continuous education group were shorter than those in routine nursing group(P<0.05).After intervention,scores of RHDS and QDTS in both groups were increased,which were higher in continuous education group than routine nursing group(P<0.05).After intervention and at 2 weeks after discharge,self-efficacy scores in both groups were increased,which were higher in continuous education group than routine nursing group(P<0.05).Conclusions Continuous education under Meleis transfer theory can accelerate recovery,improve discharge readiness and self-efficacy in patients after transurethral resection of prostatic.
