叙事护理对心脏瓣膜置换术患者创伤后成长和出院准备度的影响

Effect of Narrative Nursing on Posttraumatic Growth and Discharge Preparedness of Patients Undergoing Heart Valve Replacement Surgery

目的 探究叙事护理对心脏瓣膜置换术主要照顾者创伤后成长和出院准备度的影响。方法 选取2021年1月至2023年1月在郑州大学第二附属医院住院的92例心脏瓣膜置换术患者作为研究对象,依照患者住院时间将其分为对照组和观察组,每组46例。对照组接受围手术期常规护理,观察组基于常规护理接受叙事护理,均持续干预至出院。比较两组出院准备度、创伤后成长及心理弹性。结果 术后观察组患者的心理弹性得分高于对照组(P<0.05)。干预前,两组的创伤后成长(人生感悟、新的可能性、个人力量、与他人的关系、自我转变)得分差异无统计学意义,干预后观察组患者的创伤后成长得分高于对照组(P<0.05)。观察组患者的出院准备度(个人状态、适应能力、预期性)得分高于对照组(P<0.05)。结论 叙事护理可以提高心脏瓣膜置换术患者创伤后成长和出院准备度,提高患者的心理弹性。

Objective To explore the impact of narrative nursing on the post-traumatic growth and discharge readiness of primary caregivers undergoing heart valve replacement surgery.Methods A total of 92 patients who underwent heart valve replacement surgery and were hospitalized at the Second Affiliated Hospital of Zhengzhou University from January 2021 to January 2023 were selected as the research subjects.They were divided into a control group and an observation group based on their length of stay,with 46 patients in each group.The control group received routine perioperative care,while the observation group received narrative care based on routine care,and both continued to intervene until discharge.The readiness for discharge,post traumatic growth,and psychological resilience between the two groups were compared.Results The psychological resilience score of the postoperative observation group was higher than that of the control group(P<0.05).Before intervention,there was no statistically significant difference in the scores of post-traumatic growth(life perception,new possibilities,personal strength,relationships with others,and self transformation)between the two groups.After intervention,the observation group had a higher score of post-traumatic growth than the control group(P<0.05).The observation group had higher scores of discharge readiness(personal status,adaptability,and anticipation)compared to the control group(P<0.05).Conclusion Narrative care can improve the post-traumatic growth and discharge readiness of patients undergoing heart valve replacement surgery,and enhance their psychological resilience.
