半夏泻心汤合黄连温胆汤联合针刺治疗脾胃湿热型浅表性胃炎临床研究

Clinical Study on the Treatment of Superficial Gastritis with Spleen-Stomach Dampness-Heat Syndrome with Pinellia Heart-Draining Decoction Combined with Coptis Gallbladder-Warming Decoction and Acupuncture

目的:观察半夏泻心汤合黄连温胆汤联合针刺治疗脾胃湿热型浅表性胃炎的临床疗效。方法:将70例脾胃湿热型浅表性胃炎患者按照随机数字表法分为常规组和试验组,每组各35例。常规组给予常规针刺治疗,试验组给予半夏泻心汤合黄连温胆汤联合针刺治疗。比较两组患者治疗前后中医证候积分、肠屏障功能障碍预警指标[D-乳酸、肠脂肪酸结合蛋白(intestinal fatty acid binding protein, IFABP)、二胺氧化酶]、免疫功能指标(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8+^(+))、炎性因子水平[白细胞介素(interleukin, IL)-2、IL-6及肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)]及生活质量评分变化情况。结果:两组患者治疗后中医证候积分、D-乳酸、I-FABP及二胺氧化酶均低于本组治疗前,且治疗后试验组低于常规组(P<0.05)。两组患者治疗后CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)低于本组治疗前,CD8+高于本组治疗前,且治疗后组间比较,差异具有统计学意义(P<0.05)。两组患者治疗后IL-2、IL-6及TNF-α水平低于本组治疗前,且治疗后试验组低于常规组(P<0.05)。两组患者治疗后心理功能、生理功能、活力、躯体功能、情感职能、社会功能、精神健康及总体健康等评分均高于本组治疗前,且治疗后试验组高于常规组(P<0.05)。结论:半夏泻心汤合黄连温胆汤联合针刺治疗脾胃湿热型浅表性胃炎,可改善患者的临床症状,促进肠屏障功能恢复正常,降低炎性因子水平,提高生活质量。

Objective: To observe the clinical efficacy of Pinellia Heart-Draining Decoction combined with Coptis Gallbladder-Warming Decoction and acupuncture on superficial gastritis with spleen-stomach dampness-heat syndrome.Methods: A total of 70 patients with superficial gastritis with spleen-stomach dampness-heat syndrome were divided into the conventional group and the experimental group according to the random number table method, with 35 cases in each group.The conventional group was given conventional acupuncture treatment, while the experimental group was treated with Pinellia Heart-Draining Decoction combined with Coptis Gallbladder-Warming Decoction and acupuncture.The changes in TCM syndrome scores, intestinal barrier dysfunction early warning indexes [including D-lactic acid, intestinal fatty acid binding protein(IFABP),diamine oxidase],immune function indexes(including CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)),inflammatory factor levels [including interleukin(IL)-2,IL-6 and tumor necrosis factor-α(TNF-α)] and quality of life scores of the two groups before and after treatment were compared.Results: Results: After treatment, the TCM syndrome scores, D-lactic acid, I-FABP and diamine oxidase of the two groups were lower than those before treatment, and the experimental group′s above indexes were lower than those of the conventional group(P<0.05);The CD3^(+),CD4^(+) and CD4^(+)/CD8^(+) of the two groups were lower than those before treatment, while CD8~+ was higher than that before treatment, and all the differences between the two groups were statistically significant(P<0.05);The levels of IL-2,IL-6 and TNF-α in the two groups were lower than those before treatment, and the experimental group′s above levels were lower than those of the experimental group(P<0.05);The scores of psychological function, physiological function, vitality, physical function, emotional function, social function, mental health and general health in the two groups were higher than those before treatment, and the experimental group′s above indexes were higher than those of the experimental group(P<0.05).Conclusion: Pinellia Heart-Draining Decoction combined with Coptis Gallbladder-Warming Decoction and acupuncture on superficial gastritis with spleen-stomach dampness-heat syndrome in the treatment of superficial gastritis with spleen-stomach dampness-heat syndrome can improve the clinical symptoms of patients, promote the recovery of intestinal barrier function, reduce the level of inflammatory factors, and improve their quality of life.
