外周血COX-2、COMT、MIF水平与精神分裂症患者症状及认知功能的关系

Relationship between cyclooxygenase-2(COX-2),catechol-O-methyl transferase(COMT)and macrophage migration inhibitory factor(MIF)levels in peripheral blood and symptoms and cognitive function in patients with schizophrenia

摘要 目的 探讨精神分裂症患者外周血环氧化物酶-2(Cyclooxygenase-2,COX-2)、儿茶酚-O-甲基转移酶(Catechol-O-methyl transferase, COMT)、巨噬细胞移动抑制因子(Macrophage migration inhibitory factor, MIF)水平与其症状及认知功能的关系。方法 募集43例精神分裂症患者作为病例组,同期募集43例健康志愿者作为对照组。比较对照组与病例组血清COX-2、COMT、MIF水平。应用成套神经心理状态评估工具(Repeatable battery for the assessment of neuropsychological status, RBANS)、阳性和阴性症状量表(Positive and negative syndrome scale, PANSS)分别评估受试者的临床症状、认知功能。比较COX-2、COMT、MIF水平与受试者症状及认知功能的关系。结果 与对照组比较,病例组COX-2、COMT、MIF水平升高(P<0.05),RBANS评估中即刻记忆、空间结构、语言功能、注意力、延迟记忆五个项目的得分显著降低(P<0.05)。Pearson相关系数分析显示,COX-2水平与即刻记忆、空间结构、延迟记忆得分呈负相关,COMT水平与即刻记忆、空间结构得分呈负相关,MIF水平与五个项目得分均呈负相关(P<0.05)。病例组患者COX-2水平与阴性症状评分呈正相关,COMT水平与阴性症状、阳性症状评分呈正相关,MIF水平与炎性症状及总评分呈正相关(P<0.05)。结论 高水平COX-2、COMT、MIF与首发未服药精神分裂症患者认知损伤及病情严重程度密切相关。 Objective The aim of this study was to investigate the relationship between the levels of cyclooxygenase-2(COX-2),catechol-O-methyl transferase(COMT)and macrophage migration inhibitory factor(MIF)in peripheral blood and symptoms and cognitive function in patients with schizophrenia.Methods43 patients with schizophrenia were recruited as the case group,and 43 healthy volunteers were recruited as the control group.The serum levels of COX-2,COMT and MIF were compared between control group and case group.Repeatable Battery for the Assessment of Neuropsychological Status(RBANS)and Positive and Negative Syndrome Scale(PANSS)were used to assess the clinical symptoms and cognitive function of the subjects,respectively.The relationship between the levels of COX-2,COMT and MIF and the symptoms and cognitive function of the subjects were compared.ResultsCompared with control group,the levels of COX-2,COMT and MIF in the case group were significantly increased(all P<0.05),and the scores of immediate memory,spatial structure,language function,attention and delayed memory in RBANS assessment were significantly decreased(all P<0.05).Pearson correlation coefficient analysis showed that the level of COX-2 was negatively correlated with the scores of immediate memory,spatial structure and delayed memory;the level of COMT was negatively correlated with the scores of immediate memory and spatial structure;and the level of MIF was negatively correlated with the scores of five items(all P<0.05).In the case group,COX-2 level was positively correlated with negative symptom score;COMT level was positively correlated with negative symptom score and positive symptom score;MIF level was positively correlated with inflammatory symptom score and total score(all P<0.05).Conclu-sionHigh levels of COX-2,COMT and MIF are closely related to cognitive impairment and disease severity in first-episode drug-naive schizophrenia.
