达格列净联合沙库巴曲缬沙坦钠治疗2型糖尿病合并慢性心力衰竭患者疗效及对心功能的影响

Effect of Dapagliflozin Combined with Sacubactril Valsartan Sodium on Patients with Type 2 Diabetes Mellitus Complicated with Chronic Heart Failure and Cardiac Function

目的分析达格列净联合沙库巴曲缬沙坦钠治疗2型糖尿病合并慢性心力衰竭(chronic heart failure,CHF)患者的疗效。方法选取2020年1月—2022年12月丰县人民医院收治的80例糖尿病合并CHF患者为研究对象,按随机数表法分为两组,各40例。对照组予沙库巴曲缬沙坦钠治疗,观察组在对照组基础上加用达格列净。对比两组患者治疗前后血糖指标、心功能指标(左心室射血分数、脑钠肽)及不良反应发生情况。结果治疗后两组血糖指标(即空腹血糖和餐后2 h血糖)、血清脑钠肽均减低,且观察组的血糖指标、脑钠肽均低于对照组,差异有统计学意义(P<0.05);治疗后两组的左心室射血分数均升高,且观察组左心室射血分数高于对照组,差异有统计学意义(P<0.05);观察组总有效率为97.50%,高于对照组的77.50%,差异有统计学意义(χ^(2)=5.410,P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论联合药物方案治疗2型糖尿病合并CHF患者,对患者的血糖、心功能控制效果好,安全性高。

Objective To analyze the efficacy of dapagliflozin combined with sacubactril valsartan sodium in the treatment of type 2 diabetes mellitus patients with chronic heart failure(CHF).Methods A total of 80 patients with diabetes complicated with CHF admitted to Fengxian County People's Hospital from January 2020 to December 2022 were selected and divided into two groups according to random number table method,with 40 patients in each group.The control group was treated with sacubactril valsartan sodium,and the observation group was treated with dapagliflozin on the basis of control group.Blood glucose index,cardiac function index(left ventricular ejection fraction,brain natriuretic peptide)and adverse reactions were compared between the two groups before and after treatment.Results After treatment,blood glucose indexes(fasting blood glucose and 2-hour postprandial blood glucose)and serum brain natriuretic peptide in both groups were decreased,and the blood glucose indexes and brain natriuretic peptide in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,left ventricular ejection fraction of both groups was increased,and left ventricular ejection fraction of observation group was higher than that of control group,the difference was statistically significant(P<0.05).The total effective rate of the observation group was 97.50%,which was higher than that of the control group(77.50%),and the difference was statistically significant(χ^(2)=5.410,P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Combined drug regimen for type 2 diabetes patients with CHF has good effect on blood glucose and cardiac function control and high safety.
