血液净化治疗高甘油三酯血症性胰腺炎的疗效分析

Analysis of the efficacy of blood purification in hypertriglyceridemic acute pancreatitis

目的探讨血液净化治疗由高甘油三酯病因引起的急性胰腺炎的临床疗效。方法收集2018年9月—2022年9月徐州医科大学附属医院急诊医学科收治的150例急性胰腺炎患者,根据是否使用血液净化治疗分成两组:观察组(采用血液净化)、对照组(未采用血液净化)。在治疗48 h后,比较两组患者血清甘油三酯、血淀粉酶、炎症指标、凝血功能、血钙、乳酸等指标,以及住院时间、禁食时间和并发症发生率。结果(1)两组基线数据均无显著差异(均P>0.05);(2)两组血清甘油三酯、血淀粉酶、白细胞计数、C反应蛋白等指标均降低,且观察组较对照组变化更明显;(3)观察组住院时间、禁食时间及并发症发生率均较对照组低。结论血液净化治疗可在治疗开始后48 h内有效缓解症状,降低血清甘油三酯和炎症因子水平,缩短住院时间,减少并发症发生率。

Objective To investigate the clinical effect of blood purification in the treatment of hypertriglyceridemic acute pancreatitis(HTG-AP).Methods Collect 150 acute pancreatitis patients who were admitted to the emergency department of the Affiliated Hospital of Xuzhou Medical University from September 2018 to September 2022,according to whether to use blood purification treatment into two groups:observation group(using blood purification),and the control group(no blood purification).After 48 hours of treatment,the serum triglycerides,amylase,inflammatory markers,coagulation function,blood calcium,lactate,and the final length of hospital stay and the incidence of complications were compared.Results①No significant difference between the baseline data(both P>0.05);②In both groups,serum amylase,serum triglyceride,white blood cell count,and C-reactive protein were decreased,and the observation group changed more significantly than the control group;③The length of hospitalization,fasting duration and complication rate in the observation group were lower than those in the control group.Conclusion Blood purification therapy can effectively relieve symptoms,reduce serum triglyceride and inflammatory factor levels,shorten hospital stay,and reduce the incidence of complications within 48 hours after the start of treatment.
