基于DRGs的胃恶性肿瘤患者住院费用消耗指数灰色关联分析

Grey Correlation Analysis on Hospitalization Costs Consumption Index of Gastric Cancer on DRG

目的分析某三甲医院基于DRGs的胃恶性肿瘤患者费用消耗指数内部影响因素,为医院成本管理策略的调整提供依据和决策辅助。方法收集某三甲医院2018年1月1日-2021年9月30日共计10586例胃恶性肿瘤相关的8个DRGs分组住院费用相关数据,运用灰色关联分析法,分析胃恶性肿瘤费用消耗指数内部各因素之间的关联程度。结果8组DRGs分组中有5组RU14、RU12、GR15、RV23、GB19的药品费用消耗指数关联系数在0.9以上,有4组RU14、RU12、GR13、GB19的医技费用消耗指数关联系数在0.9以上;关联序列排前3位的依次为药品费用消耗指数0.86、医技费用消耗指数0.77、卫材费用消耗指数0.73。结论药品费用消耗指数、医技费用消耗指数是影响费用消耗指数的主要内部因素;医院可通过建立药品成本全过程管理机制、加强临床路径管理等措施实现成本管理策略的调整。

Objectives To analyze the internal influencing factors of the DRG-based cost consumption index of patients with gastric cancer in a Three A and Tertiary Hospital,and put forward reasonable suggestions to provide basis and decision support for the adjustment of hospital cost management strategy.Methods To collect the data related to the hospitalization expenses of 8 DRG groups of 10586 patients with gastric cancer from January 1st,2018 to September 30th,2021 in a Three A and Tertiary Hospital,and use the grey correlation analysis method to analyze the correlation degree between the internal factors of the cost consumption index of gastric cancer.Results Among the eight DRG groups,5 groups(RU14,RU12,GR15,RV23,GB19)had a correlation coefficient of drug cost consumption index above 0.9,and 4 groups(RU14,RU12,GR13,GB19)had a correlation coefficient of medical technology cost consumption index above 0.9;the top three in the correlation sequence were:drug cost consumption index(0.86),medical technology cost consumption index(0.77),and health materials cost consumption index(0.73).Conclusions Drug cost consumption index and medical technology cost consumption index were the main internal factors that affect the cost consumption index;the hospital should adjust the cost management strategy by establishing the whole-process management mechanism of drug cost and strengthening the clinical pathway management.
