支气管炎患儿住院费用及其影响因素

Hospitalization Expenses of Children with Bronchitis and Its Influencing Factors

目的探讨支气管炎患儿住院费用结构及影响因素,为合理控制住院费用增长提供依据。方法将2019年4月1日-2022年3月31日唐山市某医院儿科1897例支气管炎患儿作为研究对象,符合纳入标准的患儿共1000例。计算4年期间患儿住院费用结构的变化,二元logistic回归方程分析支气管炎患儿住院费用的影响因素。结果住院费用结构中药费、检查费、护理费及材料费占比较高,分别为41.53%、17.15%、13.68%及12.99%;且2019年4月1日-2020年3月31日住院费用呈上升趋势,2020年4月1日-2022年3月31日住院费用逐年下降,P<0.05;药费结构变动值及结构变动贡献率最大,分别为:2.09%、-1.28%、-1.19%;2019年4月1日-2020年3月31日住院费用成正相变动,2020年4月1日-2022年3月31日住院费用均呈负向变动;此外,年龄<8岁、住院天数≥10天、自费支付及冬季住院均为住院费用增高的影响因素,其OR值分别为:0.132、0.143、0.076、0.069、0.081,P<0.001。结论药费在住院费用结构中占较大比例,且年龄、住院天数及支付方式均为患儿住院费用的影响因素,加强医疗管控,制定有效措施,合理控制住院费用。

Objectives To explore the structure and influencing factors of hospitalization cost in children with bronchitis,and provide some basis for controlling the increase of hospitalization cost.Methods From April 1st,2019 to April 1st,2022,1897 children with bronchitis in the department of Pediatrics of a hospital in Tangshan were selected as the study objects,and 1000 children met the inclusion criteria.The structural changes of hospitalization expenses in children with bronchitis were calculated during the 4-year period,and the influencing factors of hospitalization expenses in children with bronchitis were analyzed by binary logistic regression equation.Results The cost structure of hospitalization was Chinese medicine,examination,nursing and materials,which accounted for 41.53%,17.15%,13.68%and 12.99%respectively.In addition,age<8 years old,hospitalization days≥10 days,out-of-pocket payment and hospitalization in winter were all influencing factors of increasing hospitalization costs,and the OR values were 0.132,0.143,0.076,0.069,0.081,P<0.001.Conclusion Drug cost occupies a large proportion in the structure of hospitalization cost,and age,hospitalization days and payment method were all influencing factors of hospitalization cost.Conclusions The drug cost occupied a large proportion in the structure of hospitalization cost,and age,hospitalization days and payment method were all influencing factors of hospitalization cost.
