陆家龙教授临床常用药对经验探讨

Discussion on the experience of Professor LU Jialong in treating diseases with clinically frequently used couplet medicinals

药对是中药相对固定的一种配伍,一般由2~3味中药组成,是复方配伍中最基本的单位,可增进药物的作用和疗效,扩大药物的治疗范围,减少药物的不良反应。基于药对这种方剂配伍的基本形式,在临床研究中,许许多多的灵验新方由此而衍生出来,使用适宜的药对可很大程度上增加药物的有效性,使患者在疾病的治疗中显著获益。陆家龙教授师从其父陆巨卿,为云南第一批、第二批老中医药师带徒工作指导老师,现为云南省荣誉名中医,其临证重气阴,为云南陆氏气阴学术流派带头人,从医五十余载,潜心钻研中医药理论,在临床实践中总结出了一些常见疾病的中医优势治疗方案,擅长中西医结合诊治呼吸系统疾病、心血管病、内分泌系统疾病等内科疾病,尤其在中医药治疗上述内科疾病过程中总结出了一系列经验方,处方用药整体注重气血津液。笔者经过2年的跟诊,总结学习到陆家龙教授在处方用药中擅长用对药治疗疾病,是其用药的一大特点,掌握其药对及药物组成的内在含义对运用气阴思想治疗疾病有积极的作用。文章主要通过陆家龙教授在临床上治疗消渴、眩晕、咳嗽三个疾病的常用方如加味降糖饮、加味桑芪汤、清热润燥汤来提炼总结其常用药对,并附病案加以分析说明。

Couplet medicinals is a relatively fixed compatibility of traditional Chinese medicinals,which is generally composed of 2~3 traditional Chinese medicinals,and it is the most basic unit of compound compatibility.Couplet medicinals can promote the function and efficacy of drugs,expanding the scope of the drug treatment,reduce adverse reactions.Based on the basic form of drug compatibility,many effective new prescriptions are derived in clinical research.The use of appropriate drugs can greatly increase the effectiveness of drugs,so that patients can benefit significantly in the treatment of diseases.Under the guidance of his father LU Juqing,Professor LU Jialong is the tutor for the First and Second Batch of TCM Pharmacists in Yunnan Province,and is now an honorary TCM practitioner in Yunnan Province,serving as a teacher in charge of guiding his prentices to study the science of traditional Chinese medicine.Professor LU attaches importance to Qi(气)and Yin(阴)in clinic,and he is the academic leader of the Qi and Yin School in Southwest China’s Yunnan Province.Professor LU Jialong has been practicing medicine for more than 50 years and has devoted himself to studying the theories of traditional Chinese medicine.In clinical practice,Professor LU has summarized the advantages of traditional Chinese medicine in the treatment of some common diseases.He is good at the combination of traditional Chinese medicine and Western medicine in the diagnosis and treatment of respiratory disease,cardiovascular disease,endocrine disease and other internal diseases.In particular,he has summarized a series of experience prescriptions in the treatment of these internal diseases in traditional Chinese medicine.The whole prescription pays attention to Qi,blood and body fluid.After two years of followed-up,the author concluded and learned that Professor LU made good use of couplet medicinals to treat diseases,which is a major feature of his prescription.Therefore,it is of positive significance to understand the internal meaning of traditional Chinese medicine couplet medicinals and the drug composition in the treatment of diseases based on mastering the theory of Qi and Yin.In this paper,Professor LU Jialong’s clinically frequently used couplet medicinals in clinical treatment of three diseases,namelym diabetes,vertigo and cough,with TCM decoction such as modified Jiangtang decoction(加味降糖饮),modified Sangqi decoction(加味桑芪汤)and Qingre Runzao decoction(清热润燥汤),is extracted and summarized,and medical records are attached for analysis and explanation.
