加压钢板与空心加压螺钉治疗股骨颈骨折的前瞻性随机对照研究

摘要 目的对比经皮加压钢板与空心加压螺钉在股骨颈骨折治疗中的应用价值。方法选取2022年1月—2023年1月在赣州市赣县区人民医院治疗的100例股骨颈骨折患者进行前瞻性研究,按照随机数字表法将患者分为对照组(50例)与观察组(50例),对照组开展空心加压螺钉治疗,观察组开展经皮加压钢板治疗。比较2组手术相关指标、骨折复位质量、髋关节功能及术后并发症发生情况。结果观察组较对照组手术时间、骨折愈合时间、住院时间、完全负重时间显著更短,术中出血量显著更少(P<0.05);观察组术后3个月、6个月的GardenⅠ级复位率较对照组显著更高(P<0.05);观察组术后3个月、6个月的Harris评分较对照组明显更高(P<0.05);观察组术后并发症发生率为4.00%,较对照组的18.00%低(P<0.05)。结论经皮加压钢板治疗应用于股骨颈骨折中疗效显著,不仅能提高患者的复位治疗质量,改善髋关节功能,还能促使患者尽早下地活动,缩短术后恢复时间,且并发症发生率低,值得临床推广应用。 Objective To investigate the use of compression plates versus cannulated compression screws in the treatment of femoral neck fractures.Methods A prospective study was conducted by selecting patients with femoral neck fractures treated at people's hospital of Ganxian district from January 2022 to January 2023,who were divided into control group(n=50)versus observation group(n=50)by random number table method,and control group was started on cannulated compression screw therapy,and observation group was started on percutaneous compression plate therapy.The procedure related indicators,quality of fracture reduction,hip function,and postoperative complications were observed in both groups.Results The observation group had significantly shorter operative time,fracture healing time,hospital stay,full weight bearing time and less intraoperative blood loss than the control group(P<0.05).The garden grade I reduction rate of the observation group was significantly higher(P<0.05)than that of the control group at 3 and 6 months after surgery.The Harris scores of the observation group at 3 and 6 months after surgery were significantly higher(P<0.05)than those of the control group.The postoperative complication rate of 4.00%in the observation group was significantly lower(P<0.05)than the rate of 18.00%in the control group.Conclusion Percutaneous compression plate treatment for femoral neck fractures has a significant therapeutic effect,which not only improves the reduction treatment of patients,improves the function of the hip joint,but also enables them to move down to the ground as soon as possible,shortening the postoperative recovery time,and has a low incidence of complications,which can be prioritized.
