全程专业护理个案管理对糖尿病患者自我管理效能血糖控制及生活质量的影响

The effect of full-course professional nursing case management on self-management efficiency,blood sugar control and quality of life of diabetic patients

目的探讨全程专业护理个案管理在糖尿病患者中的应用价值。方法选择2019年6月—2021年6月弋阳县中医院收治的糖尿病患者60例,按随机数字表法分为2组,每组各30例。对照组予以常规护理,观察组给予全程专业护理个案管理,均干预3个月。对比分析2组自我管理效能、糖尿病知识、血糖控制效果、生活质量。结果干预前,2组自我效能感量表(general self-efficacy scale,GSES)、密西根糖尿病知识测试问卷(Michigan diabetes knowledge test questionnaire,DKT)评分、空腹血糖(fasting blood glucose,FPG)、餐后2 h血糖(2 hours postprandial blood glucose,2 h PG)、糖化血红蛋白(glycated hemoglobin,HbA1c)水平及生活质量调查问卷(quality of life brief scale-30,QOL-30)评分相比,差异无统计学意义(P>0.05);干预后,观察组GSES、DKT、QOL-30评分高于对照组,FPG、2 h PG、HbA1c水平低于对照组,差异有统计学意义(P<0.05)。结论全程专业护理个案管理能够有效提高糖尿病患者认知,改善自我管理效能,从而平稳控糖,提升患者生活质量。

Objective To explore the application value of the whole course of professional nursing case management in diabetic patients.Methods 60 diabetic patients admitted to,Yiyang Country Traditional Chinese Medicine Hospital from June 2019 to June 2021 were selected and divided into two groups according to the random number table method,with 30 cases in each group.The control group was given routine nursing care,and the observation group was given full-course professional nursing case management,both of which intervened for 3 months.Comparative analysis of self-management efficiency,diabetes knowledge,blood sugar control effect,and quality of life between the two groups.Results Before intervention,the two groups of general self-efficacy scale(GSES),Michigan diabetes knowledge test questionnaire(DKT)scores,fasting blood glucose(FPG),postprandial 2 h blood glucose(2 h PG),glycosylated hemoglobin(HbA1c)levels and quality of life survey Compared with the questionnaire(QOL-30)scores,the difference was not statistically significant(P>0.05).After the intervention,the GSES,DKT,and QOL-30 scores of the observation group were higher than those of the control group,and the levels of FPG,2 h PG,and HbA1c were lower than those of the control group.The difference was statistically significant(P<0.05).Conclusion The whole process of professional nursing case management can effectively improve the cognition of diabetic patients,improve the efficiency of self-management,so as to control sugar steadily and improve the quality of life of patients.
