心房内心电图技术引导PICC置管在肿瘤患者中的应用

摘要 目的探讨心房内心电图技术引导经外周静脉穿刺中心静脉置管(Peripherally Inserted Central Catheter,PICC)在肿瘤患者中的应用效果。方法选取2020年1月至2021年8月我院收治的60例需行PICC置管的肿瘤患者,采用随机数字表法分为对照组和观察组各30例。对照组采用传统赛定格穿刺方法进行PICC置管,观察组采用心房内心电图技术引导PICC置管。比较两组PICC导管尖端位置、并发症发生情况。结果观察组位于上腔静脉(Superior Vena Cava,SVC)为100%、位于SVC下段及SVC与右心房交界处率为100%,均高于对照组80.00%和66.67%,差异有统计学意义(P<0.05);观察组并发症发生率为6.67%低于对照组的26.67%,差异有统计学意义(P<0.05)。结论心房内心电图技术引导PICC置管可提高肿瘤患者置管准确性,减少并发症发生,保障PICC置管安全。 Objectives To investigate the application effect of intra-atrial electrocardiogram(ECG)technique in guiding peripherally inserted central catheter(PICC)in tumor patients.Methods A total of 60 tumor patients who needed PICC catheterization admitted to our hospital from January 2020 to August 2021 were divided into control group and observation group,with 30 cases in each group.The control group received PICC catheterization by conventional Seldingerpuncture,and the observation group was treated with intra-atrial ECG techniqueto guidePICC catheterization.The position of PICC catheter tip and complications were compared between the two groups.Results The rate of observation group locating in the superior vena cava(Superior Vena Cava,SVC)was 100%,the rate of locating in the lower segment of SVC and SVC and right atrial junction was 100%,which were significantly higher than the 80.00%and 66.67%in control group(P<0.05).Conclusions PICC catheterization guided by intra-atrial ECG technique can improve the accuracy of catheterization in tumor patients,reduce the occurrence of complications,and ensure the safety of PICC catheterization.
