高强度聚焦超声治疗子宫肌瘤后再妊娠的影响因素分析

Analysis of influencing factors of pregnancy after high intensity focused ultrasound treatment of uterine fibroids

目的分析高强度聚焦超声(HIFU)治疗子宫肌瘤后再妊娠的影响因素。方法选取55例具有HIFU治疗史患者,依据是否妊娠分为妊娠组(35例)与非妊娠组(20例);妊娠组进一步分为成功分娩组(25例)与流产组(10例)。分析HIFU治疗子宫肌瘤后再妊娠的影响因素。结果妊娠组术后复发率0低于非妊娠组的15.00%,孕产史比例大于非妊娠组,差异有统计学意义(P<0.05)。经多因素Logistic回归分析显示,孕产史、术后复发为再妊娠影响因素(P<0.05)。成功分娩组年龄、肿瘤直径、辐照时间小于流产组,有孕产史比例大于流产组,差异有统计学意义(P<0.05)。经多因素Logistic回归分析显示,年龄、肿瘤直径、辐照时间、孕产史为妊娠结局影响因素(P<0.05)。结论子宫肌瘤患者行HIFU治疗后,再次妊娠与妊娠结局可受到患者年龄、孕产史、术后康复情况等因素影响。

Objective To analyze the influencing factors of pregnancy after high intensity focused ultrasound(HIFU)treatment of uterine fibroids.Methods 55 patients with HIFU treatment history were divided into pregnant group(35 cases)and non-pregnant group(20 cases)according to whether they were pregnant or not.The pregnancy group was further divided into successful delivery group(25 cases)and abortion group(10 cases).To analyze the influencing factors of pregnancy after HIFU treatment of uterine fibroids.Results The recurrence rate O of pregnancy group was lower than that of non-pregnancy group by 15.00%,and the proportion of pregnancy history was higher than that of non-pregnancy group,the difference was statistically significant(P<0.05).Multiple Logistic regression analysis showed that pregnancy history and postoperative recurrence were the influencing factors(P<0.05).The age,tumor diameter and irradiation time of the successful delivery group were smaller than those of the abortion group,and the proportion of pregnancy history was larger than that of the abortion group,with statistical significance(P<0.05).Multiple Logistic regression analysis showed that age,tumor diameter,irradiation time and gestational history were the influencing factors of pregnancy outcome(P<0.05).Conclusion After HIFU treatment,the second pregnancy and pregnancy outcome of patients with uterine fibroids can be affected by the patient's age,maternal history,postoperative rehabilitation and other factors.
