GnRH-a预处理在卵巢子宫内膜异位囊肿手术中的影响分析

Analysis of the effect of GnRH-a pretreatment in the operation of ovarian endometriosis cyst

目的分析促性腺激素释放激素激动剂(GnRH-a)预处理在卵巢子宫内膜异位囊肿手术中的影响。方法选取113例手术治疗卵巢子宫内膜异位囊肿患者,根据随机数字表法分组。对照组(56例)术前无药物预处理,试验组(57例)术前采取药物预处理,其中42例采取GnRH-a预处理(GnRH-a组),15例采取地诺孕素预处理(地诺孕素组)。所有患者均行腹腔镜卵巢子宫内膜异位囊肿剔除术,比较各组患者手术出血量、手术时间、引流管留置率、术前术后血红蛋白(Hb)下降值及术后住院时间。结果试验组术中失血量(33.25±4.33)ml显著少于对照组的(54.07±9.06)ml(P<0.05);试验组中GnRH-a组与对照组出血量比较差异有统计学意义(P<0.05),地诺孕素组与对照组比较差异无统计学意义(P>0.05)。试验组与对照组在术后住院时间比较差异无统计学意义(P>0.05)。试验组中GnRH-a组引流管留置率显著低于对照组(P<0.05)。试验组与对照组术前术后血红蛋白Hb下降值比较差异无统计学意义(P>0.05)。试验组与对照组手术时间比较差异无统计学意义(P>0.05)。结论卵巢子宫内膜异位囊肿术前采取GnRH-a预处理,可有效减少手术失血量,降低术后引流管留置率。

Objective To analyze the effect of gonadotropin-releasing hormone agonist(GnRH-a)pretreatment on ovarian endometrial cyst surgery.Methods 113 patients with ovarian endometriosis cyst were selected and grouped according to random number table method.The control group(56 cases)received no drug pretreatment before surgery,and the experimental group(57 cases)received drug pretreatment before surgery,of which 42 cases received GnRH-a pretreatment(GnRH-a group)and 15 cases received dinorgestrel pretreatment(dinorgestrel group).All patients underwent laparoscopic ovarian endometriosis cyst culling.The amount of surgical blood loss,operation time,retention rate of drainage tube,preoperative and postoperative hemoglobin(Hb)decline and postoperative hospital stay were compared among all groups.Results The intraoperative blood loss of experimental group(33.25±4.33)ml was significantly lower than that of control group(54.07±9.06)ml(P<0.05).There was significant difference in blood loss between the GnRH-a group and the control group(P<0.05),but no significant difference between the dinorgestrel group and the control group(P>0.05).There was no significant difference in postoperative hospital stay between the experimental group and the control group(P>0.05).The retention rate of drainage tube in experimental group and GnRH-a group was significantly lower than that in control group(P<0.05).There was no significant difference in Hb decline before and after operation between experimental group and control group(P>0.05).There was no significant difference in operation time between experimental group and control group(P>0.05).Conclusion Pretreatment with GnRH-a before ovarian endometriosis cyst can effectively reduce the amount of blood loss and the retention rate of postoperative drainagetube.
