不同终止妊娠时机对妊娠期糖尿病孕妇及围产儿结局的影响

Effect of different termination time of pregnancy on the outcome of pregnant women and perinatal infants with gestational diabetes

目的探讨不同终止妊娠时机对妊娠期糖尿病孕妇及围产儿结局的影响。方法选取本院120例妊娠期糖尿病孕妇为研究对象,依据不同终止妊娠时机划分为探讨组(≥39周)及对照组(<39周),比较两组围产儿并发症、孕妇分娩方式、孕妇产后2 h出血量及满意度。结果探讨组围产儿并发症发生率为5.00%,显著低于对照组的11.67%,差异有统计学意义(P<0.05);探讨组自然分娩率高于对照组,剖宫产率、产钳助产率低于对照组,差异有统计学意义(P<0.05);探讨组产后2 h出血量低于对照组,满意度高于对照组,差异有统计学意义(P<0.05)。结论针对妊娠期糖尿病孕妇血糖控制水平满意状态下,选择≥39周时终止妊娠,能够明显改善母婴结局,降低围产儿并发症发生率,减少孕妇产后2 h出血量,值得临床推广。

Objective To explore the influence of different termination time of pregnancy on the outcome of pregnant women and perinatal infants with diabetes in pregnancy.Methods 120 pregnant women with gestational diabetes in our hospital were selected as the research object,and were divided into the study group(≥39 weeks)and the control group(<39 weeks)according to different termination time of pregnancy.Perinatal complications,delivery mode of pregnant women,bleeding volume and satisfaction of pregnant women in 2 hours after delivery were compared between the two groups.Results The incidence of perinatal complications in the study group was 5.00%,significantly lower than the 11.67%in the control group,with a statistically significant difference(P<0.05);The natural delivery rate in the exploration group was higher than that in the control group,while the cesarean section rate and forceps delivery rate were lower than those in the control group,with statistically significant differences(P<0.05);The postpartum bleeding volume of the exploration group was lower than that of the control group at 2 hours,and the satisfaction level was higher than that of the control group,with a statistically significant difference(P<o.05).Conclusion For pregnant women with diabetes during pregnancy,if their blood glucose control level is satisfactory,termination of pregnancy at≥39 weeks can significantly improve the maternal and infant outcomes,reduce the incidence of perinatal complications,and reduce the amount of postpartum hemorrhage of pregnant women 2 hours,which is worthy of clinical promotion.
