摘要
目的:探究新生儿出生体重与新生儿高胆红素血症(NHB)是否存在相关性。方法:研究从重症监护医学信息数据库(MIMIC-III)提取出所有入住新生儿重症监护室(NICU)患儿的相关信息,排除非首次入住NICU、非新生儿、无出生体重信息的患儿后,最终将4882例新生儿纳入研究。根据患儿的出生体重平均分为5组,采用多因素logistic回归广义相加模型评估出生体重与NHB之间的独立相关性。结果:共4882例新生儿纳入分析,通过logistic回归模型和平滑曲线,出生体重与NHB之间存在非线性相关。患儿出生体重在1.55~3.52 kg之间,效应值为0.246,差异有统计学意义(OR=0.246;CI=0.193~0.314,P<0.001)。而体重<1.55 kg或体重>3.52 kg时体重与患病的关系,差异无统计学意义(P>0.05)。结论:研究表明,新生儿出生体重与NHB存在非线性关系,出生体重在1.55~3.52 kg之间时,体重每增加1 kg,NHB风险降低75.4%。
Objective:To investigate whether there is a correlation between neonatal birth weight and neonatal hyperbilirubinemia(NHB).Methods:Information about all children admitted to the neonatal intensive care unit(NICU)was extracted from the Medical Information on Intensive Care database(MIMIC-III),and after excluding children who were not first-time admissions to the NICU,who were not newborns,and who had no information about their birth weights,4882 newborns were finally included in the study.The children were equally divided into five groups according to their birth weight.The independent correlation between birth weight and NHB was assessed using a multifactorial logistic regression generalized summation model.Results:A total of 4882 newborns were included in the analysis.There was a nonlinear correlation between birth weight and NHB by logistic regression modeling and smoothing curves.When the birth weight of the children was between 1.55 kg and 3.52 kg,the effect value was 0.246,and the difference was statistically significant(OR=0.246;CI=0.193-0.314,P<0.001).There was no statistically significant difference in the relationship between weight and disease when weight was less than 1.55 kg or when weight was greater than 3.52 kg(P>0.05).Conclusion:There was a nonlinear relationship between neonatal birth weight and NHB,with a 75.4%reduction in the risk of NHB for each 1 kg increase in birth weight between 1.55 kg and 3.52 kg.
