无创正压通气对AECOPD并呼吸衰竭患者血气、肺动脉压力和舒适度的影响

Effects of Noninvasive Positive Pressure Ventilation on Blood Gas,Pulmonary Artery Pressure,and Comfort in Patients with AECOPD and Respiratory Failure

摘要 目的:探究无创正压通气(NIPPV)对慢性阻塞性肺病急性加重期(AECOPD)并呼吸衰竭(RF)患者血气、肺动脉压以及舒适度的影响。方法:选取2020年1月~2021年5月本院收治的80例AECOPD并RF患者,采用随机数字表法分为无创组与常规组,每组40例。常规组采用常规治疗,无创组联合NIPPV治疗,比较两组患者临床疗效、血气指标、肺动脉压力(PAP)以及舒适度。结果:无创组临床有效率为95.00%高于常规组的80.00%(P<0.05)。治疗后无创组动脉氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))水平高于常规组,动脉血二氧化碳分压(PaCO_(2))水平低于常规组(P<0.05)。治疗后无创组患者PAP水平低于常规组(P<0.05),无创组舒适状况量表(GCQ)评分高于常规组(P<0.05)。结论:NIPPV对AECOPD并RF患者的疗效确切,可显著改善患者各项血气指标,并降低患者肺动脉压力,此方式下患者舒适度有明显提高。 Objective:To explore the effects of non-invasive positive pressure ventilation(NIPPV)on blood gas,pulmonary artery pressure and comfort in patients with Chronic obstructive pulmonary disease(AECOPD)and Respiratory failure(RF).Methods:80 AECOPD patients with RF admitted to our hospital from January 2020 to May 2021 were randomly divided into a non-invasive group and a conventional group using a random number table method,with 40 patients in each group.The conventional group was treated with conventional therapy,while the non-invasive group was treated with NIPPV.The clinical efficacy,blood gas indicators,pulmonary artery pressure(PAP),and comfort of the two groups of patients were compared.Results:The clinical effective rate of the non-invasive group was 95.00%,which was higher than 80.00%in the conventional group(P<0.05).After treatment,the levels of arterial oxygen partial pressure(PaO_(2))and arterial oxygen saturation(SaO_(2))in the non-invasive group were higher than those in the conventional group,while the levels of arterial carbon dioxide partial pressure(PaCO_(2))were lower than those in the conventional group(P<0.05).After treatment,the level of PAP in the non-invasive group was lower than that in the conventional group(P<0.05),and the General Comfort Questionnaire(GCQ)score in the non-invasive group was higher than that in the conventional group(P<0.05).Conclusion:NIPPV has a definite therapeutic effect on AECOPD and RF patients,significantly improving various blood gas indicators and reducing pulmonary artery pressure.Under this method,patient comfort is significantly improved.
