维生素D滴剂联合门冬胰岛素注射液治疗妊娠糖尿病患者的临床疗效及安全性

Clinical Efficacy and Safety of Vitamin D Drops Combined with Insulin Aspartate Injection in the Treatment of Gestational Diabetes Mellitus

目的维生素D滴剂联合门冬胰岛素注射液治疗妊娠糖尿病的疗效及安全性分析。方法选取2020年1月—2022年12月龙岩市新罗区妇幼保健院收治以妊娠糖尿病为诊断的60例患者为研究对象,随机数表法分成对照组和研究组,各30例,对照组接受门冬胰岛素注射液治疗,研究组以维生素D滴剂联合门冬胰岛素注射液治疗。比较两组患者在治疗前后的血糖指标、胰岛功能指标,评定治疗效果及用药不良反应,统计随访不良妊娠结局。结果治疗后两组血糖指标水平及胰岛功能中胰岛素抵抗指数(homeostatic model assessment-insulin resistance,Homa-IR)较治疗前明显下降,研究组下降程度更明显,而胰岛功能中胰岛素分泌指数(homeostasis model assessment-β,Homa-β)较治疗前提升,研究组提升程度更明显,差异有统计学意义(P<0.05)。两组不良妊娠结局率相比,差异无统计学意义(P>0.05)。结论妊娠糖尿病患者治疗中,以维生素D滴剂联合门冬胰岛素注射液开展治疗可降低血糖指标水平改善胰岛功能指标,提升治疗效果,较为理想。

Objective To observe the efficacy and safety of vitamin D drops combined with insulin aspartate injection in the treatment of gestational diabetes.Methods A total of 60 patients diagnosed with gestational diabetes were ad-mitted to Longyan Xinluo District Maternal and Child Health Hospital from January 2020 to December 2022,and they were divided into the control group and the study group with 30 cases in each group by random number table method.The control group received insulin aspartate injection,and the study group received vitamin D drops combined with in-sulin aspartate injection.Blood glucose indexes and islet function indexes of the two groups were compared before and after treatment,the therapeutic effect and adverse drug reactions were evaluated,and adverse pregnancy outcomes were statistically followed up.Results After treatment,the levels of blood glucose indexes and homeostatic model as-sessment insulin resistance(homa-IR)indexes in islet function decreased significantly compared with that before treatment,especially in the study group.Islet function homeostasis model assessment-β(homa-β)was higher than be-fore treatment,and the improvement was more obvious in the study group,and the differences were statistically signifi-cant(P<0.05).There was no significant difference in the rate of adverse pregnancy outcome(P>0.05).Conclusion In the treatment of gestational diabetes mellitus patients,vitamin D drops combined with insulin aspartate injection can significantly reduce the level of blood glucose indicators,improve the islet function indicators,and enhance the thera-peutic effect,which is relatively ideal.
