宣肺败毒颗粒联合无创正压通气治疗慢性阻塞性肺疾病急性加重期伴Ⅱ型呼吸衰竭患者的效果

Effects of Xuanfei Baidu granules combined with noninvasive positive pressure ventilation in treatment of patients with acute exacerbation of chronic obstructive pulmonary disease combined with type II respiratory failure

目的:观察宣肺败毒颗粒联合无创正压通气(NPPV)治疗慢性阻塞性肺疾病急性加重期(AECOPD)伴Ⅱ型呼吸衰竭患者的效果。方法:选取2022年2月至2023年1月该院收治的120例AECOPD伴Ⅱ型呼吸衰竭患者进行前瞻性研究,根据随机数字表法将其分为观察组与对照组各60例。对照组采用NPPV治疗,观察组在对照组基础上联合宣肺败毒颗粒治疗,比较两组临床疗效、治疗前后血气分析指标[动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2))]水平、炎性因子[白细胞介素-17(IL-17)、白细胞介素-18(IL-18)]水平和不良反应发生率。结果:观察组治疗总有效率为86.67%(52/60),高于对照组的60.00%(36/60),差异有统计学意义(P<0.05);治疗后,两组PaCO_(2)水平均低于治疗前,且观察组低于对照组,两组PaO_(2)水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);两组IL-17、IL-18水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:宣肺败毒颗粒联合NPPV治疗AECOPD伴Ⅱ型呼吸衰竭患者可提高治疗总有效率,改善血气分析指标水平,降低炎性因子水平,其效果优于单纯NPPV治疗。

Objective:To observe effects of Xuanfei Baidu granules combined with noninvasive positive pressure ventilation(NPPV)in treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)combined with type II respiratory failure.Methods:A prospective study was conducted on 120 patients with AECOPD combined with type II respiratory failure admitted to the hospital from February 2022 to January 2023.According to the random number table method,they were divided into observation group and control group,60 cases in each group.The control group was treated with NPPV,while the observation group was treated with Xuanfei Baidu granules on the basis of that of the control group.The clinical efficacy,the blood gas analysis indexes[arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2))]levels before and after the treatment,the inflammatory factors[interleukin-17(IL-17),interleukin-18(IL-18)]levels,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 86.67%(52/60),which was higher than 60.00%(36/60)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the PaCO_(2) levels of the two groups were lower than those before the treatment,and that in the observation group was lower than that in the control group;the PaO_(2) levels of the two groups were higher than those before the treatment,and that in the observation group was higher than that in the control group;and the differences were statistically significant(P<0.05).The levels of IL-17 and IL-18 in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Xuanfei Baidu granules combined with NPPV in the treatment of the AECOPD patients combined with type II respiratory failure can improve the total effective rate of treatment,improve the levels of blood gas analysis indexes,and reduce the levels of inflammatory factors.Moreover,it is superior to single NPPV treatment.
