摘要
目的:分析血清热休克蛋白90(HSP90)、可溶性程序性死亡受体1(sPD-1)、可溶性尿激酶型纤溶酶原激活物受体(suPAR)水平联合检测在原发性肾病综合征(PNS)发作期患儿诊断中的效能。方法:回顾性分析2020年5月至2022年5月该院收治的80例PNS患儿的临床资料,根据疾病活动度将其分为发作期组(n=40)、缓解期组(n=40),另选取同期40名健康体检儿童作为对照组。比较三组血清HSP90、sPD-1、suPAR水平,采用Spearman相关性分析血清HSP90、sPD-1、suPAR水平与疾病活动度的相关性,绘制受试者工作特征曲线(ROC)分析血清HSP90、sPD-1、suPAR水平单项及联合检测诊断PNS患儿发作期的效能。结果:发作期组血清HSP90、sPD-1、suPAR水平均高于缓解期组和对照组,且缓解期组高于对照组,差异有统计学意义(P<0.05);Spearman相关性分析结果显示,血清HSP90、sPD-1、suPAR水平与疾病活动度均呈正相关(r>0,P<0.05);ROC曲线分析结果显示,血清HSP90、sPD-1、suPAR水平单项及联合检测诊断PNS发作期患儿的曲线下面积分别为0.725、0.625、0.675、0.875,均具有一定诊断效能,且联合检测诊断效能最佳。结论:血清HSP90、sPD-1、suPAR水平联合检测诊断PNS发作期患儿的效能均高于三者单项检测。
Objective:To analyze efficiency of combined detection of serum heat shock protein 90(HSP90),soluble programmed death receptor 1(sPD-1)and soluble urokinase-type plasminogen activator receptor(suPAR)levels in diagnosis of children with primary nephrotic syndrome(PNS)in attack period.Methods:The clinical data of 80 children with PNS admitted to this hospital from May 2020 to May 2022 were retrospectively analyzed.According to the disease activity,they were divided into attack group(n=40)and remission group(n=40).Another 40 healthy children in the same period were selected as the control group.The levels of serum HSP90,sPD-1 and suPAR were compared among the three groups.Using Spearman correlation analysis to investigate the correlation between HSP90,sPD-1,suPAR and disease activity.The receiver operating characteristic curve(ROC)was drawn to analyze the efficiencies of single and combined detection of HSP90,sPD-1 and suPAR levels in the diagnosis of onset in children with PNS in the attack period.Results:The levels of serum HSP90,sPD-1 and suPAR in the attack group were higher than those in the remission group,those in the remission group were higher than those in the control group,and the differences were statistically significant(P<0.05).The Spearman correlation analysis showed that the serum HSP90,sPD-1,suPAR levels were positively correlated with the disease activity(r>0,P<0.05).The ROC curve showed that the areas under the curve of single and combined detection of serum HSP90,sPD-1 and suPAR levels in the diagnosis of children with PNS in the attack period were 0.725,0.625,0.675 and 0.875,separately,all of which had certain diagnostic efficiencies,and the combined detection had the highest diagnostic efficiency.Conclusions:The efficiency of combined detection of serum HSP90,sPD-1 and suPAR levels in the diagnosis of children with PNS in the attack period is higher than that of single detection of the three.
