尿微量白蛋白/肌酐、血肌酐、转铁蛋白水平联合检测在糖尿病早期肾功能损伤诊断中的效能 被引量:1

摘要 目的:分析尿微量白蛋白/肌酐比值(UACR)、血肌酐(Scr)、转铁蛋白(TRF)水平联合检测在糖尿病早期肾功能损伤诊断中的效能。方法:回顾性分析2022年3月至2023年3月商丘仁和中医院收治的100例糖尿病患者的临床资料,设为观察组;根据有无早期肾功能损伤将其分为损伤组(n=44)和未损伤组(n=56);另选取同期100名健康体检者的临床资料,设为对照组。比较三组UACR、Scr、TRF水平,并绘制受试者工作特征(ROC)曲线,分析UACR、Scr、TRF水平单项及联合检测诊断糖尿病早期肾功能损伤的效能。结果:损伤组UACR、Scr水平均高于未损伤组、对照组,且未损伤组高于对照组,损伤组TRF水平均低于未损伤组、对照组,且未损伤组低于对照组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,UACR、Scr、TRF水平单项及联合检测诊断糖尿病早期肾功能损伤的曲线下面积分别为0.893、0.876、0.891、0.994,均具有一定诊断效能,其中联合检测诊断效能最高。结论:UACR、Scr、TRF水平联合检测在糖尿病早期肾功能损伤诊断中的效能高于三者单项检测。 Objective:To analyze efficiency of combined detection of urinary microalbumin/creatinine ratio(UACR),serum creatinine(Scr)and transferrin(TRF)levels in diagnosis of early renal function injury in patients with diabetes.Methods:The clinical data of 100 diabetic patients admitted to Shangqiu Renhe Hospital of Traditional Chinese Medicine from March 2022 to March 2023 were retrospectively analyzed and they were set as the observation group.According to the presence or absence of early renal function injury,they were divided into injury group(n=44)and noninjury group(n=56).The clinical data of 100 healthy subjects in the same period were retrospectively analyzed and they were selected as the control group.The levels of UACR,Scr and TRF were compared among the three groups.The receiver operating characteristic(ROC)curve was drawn to analyze the efficiencies of single and combined detection of UACR,Scr and TRF levels in the diagnosis of early renal function injury in the patients with diabetes.Results:The levels of UACR and Scr in the injury group were higher than those in the non-injury group and the control group,and those in the non-injury group were higher than those in the control group;the level of TRF in the injury group was lower than that in the non-injury group and the control group,and that in the non-injury group was lower than that in the control group;and the differences were statistically significant(P<0.05).The ROC curve showed that the AUCs of single and combined detection of UACR,Scr and TRF levels in the diagnosis of early renal function injury in the patients with diabetes were 0.893,0.876,0.891 and 0.994,separately,all of which had certain diagnostic efficiency,and the combined detection had the highest diagnostic efficiency.Conclusions:The combined detection of UACR,Scr and TRF levels is more effective in the diagnosis of early renal function injury in the patients with diabetes than the single detection of the three.
