头针长留针治疗脑卒中后平衡功能障碍疗效的Meta分析

Effect of long-time retention of scalp needle in the treatment of balance dysfunction after stroke:a meta-analysis

目的系统评价头针长留针治疗脑卒中后平衡功能障碍的临床疗效。方法检索PubMed、中国知网、维普、万方数据库中头针长留针治疗脑卒中后平衡功能障碍的随机对照试验,检索时间从建库至2022年10月。采用Cochrane手册对纳入文献进行质量评价,利用RevMan5.4软件进行Meta分析。结果最终纳入21篇文献,共涉及1711例患者,其中研究组870例、对照组841例。Meta分析结果显示,研究组的Berg平衡量表评分(MD=6.31,95%CI:4.97~7.65,Z=9.24,P<0.00001)、有效率(RR=1.21,95%CI:1.12~1.31,Z=4.88,P<0.00001)、Fugl-Meyer运动功能量表评分(MD=9.72,95%CI:6.60~12.38,Z=6.12,P<0.00001)、Barthel指数评分(MD=9.54,95%CI:5.36~13.72,Z=4.47,P<0.00001)、改良Barthel指数评分(MD=6.78,95%CI:4.10~9.46,Z=4.96,P<0.00001)以及Fugl-Meyer平衡量表评分(MD=2.25,95%CI:0.57~3.93,Z=2.63,P=0.009)均优于对照组。结论头针长留针治疗脑卒中后平衡功能障碍具有一定疗效,能有效改善脑卒中患者平衡功能,提高运动和日常生活能力。

Objective To systematically evaluate the therapeutic effect of long-time retention of scalp acupuncture on balance dysfunction after stroke.Methods Randomized controlled trial(RCT)of long-time retention of scalp needle in the treatment of balance dysfunction after stroke were searched in PubMed,CNKI,VIP and WanFang Data.The retrieval time was from the establishment of the database to October 2022.Cochrane manual was used to evaluate the quality of the included literature,and RevMan5.4 software was used for meta-analysis.Results Finally,21 articles were included,involving a total of 1711 patients.The results of meta-analysis showed that the Berg Balance Scale Score of the longtime retention of scalp needle group(MD=6.31,95%CI:4.97-7.65,Z=9.24,P<0.00001),clinical efficacy evaluation(RR=1.21,95%CI:1.12-1.31,Z=4.88,P<0.00001),FMA score(MD=9.72,95%CI:6.60-12.38,Z=6.12,P<0.00001),Barthel index(MD=9.54,95%CI:5.36-13.72,Z=4.47,P<0.00001),the improved Barthel index(MD=6.78,95%CI:4.10-9.46,Z=4.96,P<0.00001)and FMB score(MD=2.25,95%CI:0.57-3.93,Z=2.63,P=0.009)were more excellent than the control group.Conclusion Long-time retention of scalp needle has a certain effect in treating balance dysfunction after stroke,which can effectively improve the balance function of stroke patients,improve walking ability and daily living ability.
