摘要
目的探讨上海市崇明区老年人群白蛋白尿的影响因素。方法2011年5月—2011年10月,对上海市崇明区城桥镇居民进行随机整群抽样。根据尿微量白蛋白/尿肌酐比值(UACR)≥30 mg/g确定白蛋白尿组473例,采用倾向评分1∶3匹配正常组1419例。记录受试者的年龄、性别、体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)和腰臀比(WHR)。检测受试者的总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FBG)、餐后2 h血糖(2 hPBG)、糖化血红蛋白(HbA1c)、血尿酸(UA)和估算肾小球滤过率(eGFR)。采用相关分析对各因素与UACR的相关性进行分析,二元logistic回归法分析UACR的影响因素。结果与正常组相比,白蛋白尿组BMI、SBP、DBP、TG、LDL-C、FBG、2 hPBG、HbA1c和UA水平较高,eGFR水平较低,差异均有统计学意义(P<0.05)。相关性分析显示,BMI、SBP、DBP、TG、LDL-C、FBG、2 hPBG、HbA1c和UA与UACR呈正相关;eGFR与UACR呈负相关。二元logistic回归分析显示,BMI≥28 kg·m^(-2),SBP、LDL-C和FBG升高为UACR的危险因素,eGFR升高为UACR的保护因素。结论BMI≥28 kg·m^(-2),SBP、LDL-C、FBG升高和eGFR降低可能是上海市崇明区老年人群白蛋白尿的影响因素。
Objective To explore the influencing factors of albuminuria in elderly population in Chongming district of Shanghai city.Methods From May 2011 to October 2011,a random cluster sampling was conducted among the residents of Chengqiao Town,Chongming District,Shanghai.According to urinary microalbumin/urinary creatinine ratio(UACR)≥30 mg/g,473 subjects were included in albuminuria group.A propensity score of 1∶3 was used to match 1419 subjects in the normal group.Their age,gender,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),waist-to-hip ratio(WHR)were recorded.Their total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),fasting blood glucose(FBG),postprandial 2-hour blood glucose(2 hPBG),glycated hemoglobin(HbA1c),blood uric acid(UA)and estimated glomerular filtration rate(eGFR)were measured.The correlation analysis was used to analyze the correlation between each factor and UACR,and binary logistic regression was used to analyze the influencing factors of UACR.Results Compared with the normal group,the levels of BMI,SBP,DBP,TG,LDL-C,FBG,2 hPBG,HbA1c and UA of the albuminuria group were higher,and the level of eGFR was lower,with statistical significances(P<0.05).Correlation analysis showed that BMI,SBP,DBP,TG,LDL-C,FBG,2 hPBG,HbA1c and UA were positively correlated with UACR,and eGFR was negatively correlated with UACR(P<0.05).Binary logistic regression analysis showed that BMI≥28 kg·m^(-2) and elevated SBP,LDL-C and FBG were risk factors for UACR,while elevated eGFR was a protective factor for UACR.Conclusion BMI≥28 kg·m^(-2),elevated SBP,LDL-C,FBG,and decreased eGFR may be influencing factors for albuminuria in the elderly population in Chongming district of Shanghai city.
