加减通窍活血汤治疗突发性耳聋的临床效果探讨

Clinical Effect of Modified Tongqiaohuoxue Decoction on Sudden Deafness

目的探讨在突发性耳聋治疗中,选择加减通窍活血汤的价值。方法采集2021年1月—2023年3月在无锡市惠山区中医医院确诊为突发性耳聋的88例患者资料,以随机数表法分组,对照组(44例)接受西医治疗,观察组(44例)在西医治疗基础上接受加减通窍活血汤治疗。比较两组的临床疗效、临床指标、临床症状改善时间、凝血功能及不良反应发生率。结果观察组临床疗效(95.45%)高于对照组(81.82%),差异有统计学意义(χ^(2)=4.061,P=0.043);观察组不良反应发生率与对照组相比,差异无统计学意义(P>0.05);观察组听力损伤评分、纯音测听阈值低于对照组,观察组听力、耳鸣、耳闷、眩晕改善时间短于对照组,差异有统计学意义(P<0.05);观察组凝血酶时间、纤维蛋白原、凝血酶原时间、活化部分凝血活酶时间低于对照组,差异有统计学意义(P<0.05)。结论在突发性耳聋治疗中,选择加减通窍活血汤可改善其临床症状及凝血功能,亦可减轻听力损伤程度,确保治疗效果及安全性。

Objective To explore the value of modified Tongqiaohuoxue decoction in the treatment of sudden deafness.Methods The data of 88 patients diagnosed with sudden deafness in Huishan District Hospital of Traditional Chinese Medicine from January 2021 to March 2023 were collected,and divided into two groups by random number table method.The control group(44 cases)received western medicine treatment in one combination,and the observation group(44 cases)received Tongqiaohuoxue decoction on the basis of western medicine treatment.The clinical efficacy,clinical indicators,improvement time of clinical symptoms,coagulation function and incidence of adverse reactions were compared between the two groups.Results The clinical effect of the observation group(95.45%)was higher than that of the control group(81.82%),and the difference was statistically significant(χ^(2)=4.061,P=0.043).There was no statistically significant difference of the incidence of adverse reactions in observation group compared with control group(P>0.05).The hearing impairment score and pure tone hearing threshold in the observation group were lower than those in the control group,and the improvement time of hearing,tinnitus,stuffy ear and vertigo in observation group were shorter than those in control group,and the differences were statistically significant(P<0.05).The thrombin time,fibrinogen time,prothrombin time and activated partial thromboplastin time in observation group were lower than those in control group,and the differences were statistically significant(P<0.05).Conclusion In the treatment of sudden deafness,the selection of modified Tongqiaohuoxue decoction can improve the clinical symptoms and coagulation function,but also reduce the degree of hearing damage,ensure the treatment effect and safety.
