摘要
目的探究急性脑梗死患者治疗中丁苯酞结合阿替普酶静脉溶栓的临床应用效果及对出血性转化的影响。方法选取2021年10月—2022年3月江苏省东海县人民医院急诊科收治的112例急性脑梗死患者为研究对象,以信封法分为观察组和对照组,各56例。对照组接受阿替普酶静脉溶栓治疗,观察组在对照组基础上联合应用丁苯酞治疗。对比两组患者氧化应激指标、血管内皮细胞水平、血液流变学指标以及出血性转化率。结果观察组超氧化物歧化酶、丙二醛、一氧化氮、内皮素-1水平均较对照组更优,差异有统计学意义(P<0.05);观察组血浆黏度、全血低切黏度、全血高切黏度均较对照组更低,差异有统计学意义(P<0.05);观察组出血性转化率为5.36%比对照组的17.86%更低,差异有统计学意义(χ^(2)=4.264,P<0.05)。结论丁苯酞结合阿替普酶静脉溶栓可以有效提高急性脑梗死治疗效果,控制出血性转化现象。
Objective To explore the clinical effect of intravenous thrombolytic therapy of butylphthalein combined with alteplase in patients with acute cerebral infarction and its influence on hemorrhagic transformation.Methods 112 patients with acute cerebral infarction admitted to the Emergency Department of Donghai County People's Hospital in Jiangsu Province from October 2021 to March 2022 were selected as the study subjects.They were divided into an ob‐servation group and a control group using envelope method,with 56 cases in each group.The control group received intravenous thrombolysis with ateplase,while the observation group received combined treatment with butylphthalide on the basis of the control group.Compared oxidative stress indicators,endothelial cell levels,hemorheological indica‐tors,and hemorrhagic transformation rate between two groups of patients.Results The levels of superoxide dismutase,malondialdehyde,nitric oxide,and endothelin-1 in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).The plasma viscosity,whole blood low shear viscosity,and whole blood high shear viscosity of the observation group were all lower than those of the control group,andthe differ‐ence was statistically significant(P<0.05).The hemorrhagic conversion rate of the observation group was 5.36%,which was lower than the control group's 17.86%,and the difference was statistically significant(χ^(2)=4.264,P<0.05).Conclusion Intravenous thrombolysis with alteplase combined with butylphthalein can effectively improve the thera‐peutic effect of acute cerebral infarction and control the hemorrhagic transformation phenomenon.
