单纯结扎阑尾残端在腹腔镜阑尾切除术中的应用价值分析

Analysis of the Application Value of Simple Ligation of Appendiceal Stump in Laparoscopic Appendectomy

目的分析单纯结扎阑尾残端在腹腔镜阑尾切除术中的应用价值。方法选取2022年5月—2023年7月于东莞市凤岗医院接受阑尾手术的70例患者为研究对象。按随机数表法分为对照组(n=35)和观察组(n=35)。对照组采取荷包缝合方式进行阑尾残端处理,观察组实施单纯结扎阑尾残端。对比两组患者的临床治疗的效果。结果对照组术后肛门首次通气时间、术后住院时间短于观察组,但观察组手术时长、术中失血量、术后疼痛程度优于对照组,差异有统计学意义(P<0.05)。手术前,两组患者CD4+/CD8+、CD3+水平对比,差异无统计学意义(P>0.05);手术后,两组患者CD4+/CD8+、CD3+水平均有所下降,但观察组患者上述指标明显高于对照组,差异有统计学意义(P<0.05)。相较于对照组的17.14%,观察组患者术后并发症总发生率(2.86%)明显更低,差异有统计学意义(χ^(2)=3.968,P<0.05)。结论单纯结扎阑尾残端在腹腔镜阑尾切除术中可缩短手术时间、降低术中出血量,患者术后疼痛度较低有利于患者身心健康。

Objective To analyze the application value of simple ligation of appendiceal stump in laparoscopic appen‐dectomy.Methods A total of 70 patients who underwent appendectomy from May 2022 to July 2023 in Dongguan Fenggang Hospital were selected as the study objects.They were divided into the control group(n=35)and the obser‐vation group(n=35)according to the method of random number table.The control group adopted the purse-string su‐ture method for appendiceal stump treatment,and the observation group implemented simple ligation of the appendi‐ceal stump.The effects of clinical treatment of the two groups were compared.Results The postoperative anal first ven‐tilation time and postoperative hospitalization time of the control group were shorter than those of the observation group,operation duration,intraoperative blood loss and postoperative pain of the observation group were better than those of the control group,and the differences were statistically significant(P<0.05).Before surgery,there were no sta‐tistically significant difference in the levels of CD4+/CD8+and CD3+between the two groups(P>0.05).After surgery,the levels of CD4+/CD8+and CD3+decreased in both groups,but the indexes of the patients in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Compared with 17.14%in the control group,the total incidence of postoperative complications in the observation group(2.86%)was lower,and the difference was statistically significant(χ^(2)=3.968,P<0.05).Conclusion Simple liga tion of the appendiceal stump in laparoscopic appendectomy can shorten the operation time,reduce intraoperative bleeding,and the patients'lower postoperative pain is conducive to their physical and mental health.
