钢板螺钉内固定及早期负重治疗内外踝骨折的疗效分析

Effect Analysis of Internal Fixation with Plate Screw and Early Weight Bearing in the Treatment of Internal and External Malleolar Fractures

目的研究内外踝骨折采取钢板螺钉固定及早期负重方案的安全性。方法选取2019年2月—2022年1月上海交通大学医学院苏州九龙医院收治的外踝骨折患者70例,将患者分为早期负重组和非早期负重组,每组35例,早期负重组允许患者在术后3周开始部分负重,非早期负重组患者保持6周不负重。比较两组骨性愈合情况、术后并发症、踝关节功能评分及生活质量评分。结果两组骨性愈合情况及术后并发症比较,差异无统计学意义(P>0.05)。术后12周、6个月、1年,早期负重组的踝关节功能评分高于非早期负重组,差异有统计学意义(P<0.05)。术后8周、12周、6个月、1年,早期负重组的生活质量评分高于非早期负重组,差异有统计学意义(P<0.05)。结论钢板螺钉内固定治疗内外踝骨折的早期负重是安全的,且可以从早期负重治疗方案中受益。

Objective To study the safety of plate screw fixation and early weight-bearing regimen for internal and ex⁃ternal malleolar fractures.Methods Seventy patients with lateral ankle fracture admitted to Suzhou Jiulong Hospital of Shanghai Jiaotong University School of Medicine from February 2019 to January 2022 were selected and divided into early negative recombination and non-early negative recombination,with 35 cases in each group.Early negative re⁃combination patients started partial weight bearing 3 weeks after surgery,while non-early negative recombination pa⁃tients remained weight-free for 6 weeks.Bone healing,postoperative complications,ankle joint function score and quality of life score were compared between the two groups.Results There was no statistically significant difference in bone healing and postoperative complications between the two groups(P>0.05).At 12 weeks,6 months and 1 year af⁃ter surgery,the ankle joint function score of early negative recombination was higher than that of non-early negative recombination,and the difference was statistically significant(P<0.05).At 8 weeks,12 weeks,6 months and 1 year af⁃ter surgery,the quality of life score of early negative reorganization was higher than that of non-early negative reorga⁃nization,and the difference was statistically significant(P<0.05).Conclusion Early weight-bearing in the treatment of internal and external malleolar fractures with plate screw fixation is safe and may benefit from an early weight-bearing regimen.
