内镜下巴曲亭局部喷洒联合泮托拉唑治疗消化性溃疡出血的临床观察

Curative Effect of Endoscopic Local Spraying of Hemocoagulase for Injection Combined with Pantoprazole in Peptic Ulcer Bleeding

目的探究内镜下巴曲亭局部喷洒联合泮托拉唑治疗消化性溃疡出血的临床治疗效果。方法将2020年1月—2023年5月收治的60例消化性溃疡出血患者作为研究对象。使用数字随机法将上述患者分为研究组和对照组,两组各30例。对照组患者接受泮托拉唑治疗,研究组患者接受内镜下巴曲亭局部喷洒联合泮托拉唑治疗,治疗周期为7 d。对比两组患者的治疗效果,观察治疗后两组患者临床症状(反酸、大便潜血、呕血)发生情况,对比两组患者治疗前后凝血功能[凝血酶时间(TT)、凝血酶原时间(PT)、血清纤维原蛋白(FIB)]、胃功能指标[胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、胃泌素(G-17)],记录两组患者治疗期间不良反应(皮疹、腹泻、头痛)发生情况。结果研究组患者的治疗有效率为93.33%,高于对照组的73.33%(P<0.05);治疗后,研究组患者的临床症状发生率为16.67%,低于对照组的40.00%(P<0.05);研究组患者的TT、PT、FIB水平均低于对照组(P<0.05);研究组患者PGⅠ、PGⅡ水平低于对照组,G-17水平高于对照组(P<0.05);研究组治疗期间不良反应发生率为23.33%,对照组为13.33%,两组患者差异无统计学意义(P>0.05)。结论内镜下巴曲亭局部喷洒联合泮托拉唑能够提高消化性溃疡出血患者的治疗效果,有效减少临床症状的发生率,改善患者的凝血功能,降低炎症反应,且治疗期间不良反应发生率较低,能够满足临床治疗需求。

Objective To explore the clinical curative effect of endoscopic local spraying of hemocoagulase for injection combined with Pantoprazole in peptic ulcer bleeding.Methods A total of 60 patients with peptic ulcer bleeding admitted to the hospital were enrolled as the research objects between January 2020 and May 2023.According to random number table method,they were divided into study group and control group,30 cases in each group.The control group was treated with Pantoprazole,while study group was additionally treated with endoscopic local spraying of hemocoagulase for injection.All were treated for 7 d.The curative effect in the two groups was compared.The occurrence of clinical symptoms(acid reflux,occult blood in stool,hematemesis)in the two groups was observed after treatment.The coagulation function indexes[thrombin time(TT),prothrombin time(PT),serum fibrinogen(FIB)]and gastric function indexes[pepsinogenⅠ(PG-Ⅰ),pepsinogenⅡ(PG-Ⅱ),gastrin-17(G-17)]in the two groups were compared before and after treatment.The occurrence of adverse reactions(rash,diarrhea,headache)in the two groups was recorded during treatment.Results The response rate of treatment in study group was higher than that of control group(93.33%vs 73.33%,P<0.05).After treatment,incidence of clinical symptoms in study group was lower than that in control group(16.67%vs 40.00%,P<0.05).The levels of TT,PT and FIB in study group were lower than those in control group(P<0.05).The levels of PG-Ⅰand PG-Ⅱin study group were lower than those in control group,while G-17 was higher than that in control group(P<0.05).There was no significant difference in incidence of adverse reactions between study group and control group during treatment(23.33%vs 13.33%,P>0.05).Conclusion Endoscopic local spraying of hemocoagulase for injection combined with Pantoprazole can improve the curative effect in patients with peptic ulcer bleeding,effectively reduce the incidence of clinical symptoms,improve coagulation function,reduce inflammatory response and incidence of adverse reactions during treatment,which can meet the clinical treatment demands.
