加减桃红四物汤联合低分子肝素钙对下肢骨折术后深静脉血栓发生率、血流变及凝血功能的影响

Effects of Modified Taohong Siwu Decoction combined with low molecular weight heparin calcium on the incidence,blood rheology and coagulation functionof patients after lower limb fracture

目的 观察加减桃红四物汤联合低分子肝素钙治疗对下肢骨折术后患者深静脉血栓发生率、血流变及凝血功能的影响。方法 将我院下肢骨折术后患者84例随机分为加减桃红四物汤联合低分子肝素钙治疗组(联合治疗组)与低分子肝素钙单独治疗组(单独治疗组)各42例,对两组深静脉血栓发生情况、血液流变学(血浆黏度、高切全血黏度、低切全血黏度、红细胞压积)、凝血功能[凝血酶原时间(PT)、纤维蛋白原(FIB)、D-二聚体(D-D)]进行统计分析。结果 治疗后,联合治疗组深静脉血栓发生率显著低于单独治疗组(P<0.05);两组血浆黏度、高切、低切全血黏度、红细胞压积、PT、FIB、D-D水平与治疗前比较差异均有统计学意义(P<0.05),且联合治疗组优于单独治疗组(P<0.05)。结论 加减桃红四物汤联合低分子肝素钙可有效降低下肢骨折术后患者深静脉血栓发生率,促进患者恢复,值得临床推广,其作用机制可能为改善患者的血流变及凝血功能相关指标。

Objective To observe the effects of Modified Taohong Siwu Decoction combined with low molecular weight heparin calcium therapy on the incidence,blood rheology and coagulation function of patients after lower limb fracture.Methods Eighty-four patients with lower limb fractures were randomly divided into single treatment group and combination group,42 cases each.Single treatment group received low molecular weight heparin calcium,while combination group received Modified Taohong Siwu Decoction on the basis of single treatment group,and the deep vein thrombosis,blood rheology(plasma viscosity,high cut whole blood viscosity,low cut whole blood viscosity,erythrocyte mass),coagulation function[prothrombin time(PT),fibrinogen(FIB),D-dimer(D-D)]in the two groups were statistically analyzed.Results After treatment,the incidence of deep vein thrombosis in combination group was significantly lower than that in treatment group(P<0.05).The plasma viscosity,high cut and low cut whole blood viscosity,hematocrit,PT,FIB,D-D levels between the two groups were statistically significant compared with before treatment(P<0.05),and combination treatment group was better than single treatment group(P<0.05).Conclusion Modified Taohong Siwu Decoction combined with low molecular weight heparin calcium can effectively reduce the incidence of deep vein thrombosis in patients after lower limb fracture surgery,promote the recovery of patients,and its mechanism of action might be improving the blood rheology and coagulation function related indicators of patients.
